日本語
末期腎不全患者では動脈硬化が加速度的に進展し、それに伴い心血管病変(CVD)の発症頻度が高く、患者予後にも大きく影響を及ぼしており、それら病態の把握は急務であると同時に早期に進展因子の評価を行うことは需要な意味を持つ。本研究の目的は、一般的な動脈硬化危険因子と腎不全患者特有の動脈硬化関連因子が動脈硬化進展にどのように関与するかを検討し、動脈硬化進展に対する予測因子を解析することにある。

Atherosclerotic cardiovascular disease (CVD) is characterized by accumulation of lipids, inflammatory cells (macrophages, lymphocytes and smooth muscles cells) and extracellular matrix in the plaque, and inflammation plays a central role in the atherogenetic process. Moreover end-stage renal disease (ESRD) patients present a characteristic atherogenic state as mineral bone disorder and oxidative stress enhancement. Those states may induce high prevalence of CVD events, and CVD contributes significantly to the markedly increased mortality in these patients. The purpose of the study is to estimate how inflammation, oxidative stress and vascular calcification associated factors affect progress of atherosclerosis in ESRD patients.

一般的な動脈硬化危険因子である脂質代謝異常、高血圧、栄養状態と腎不全に関連した動脈硬化進展因子として炎症性因子(IL6, CRPなど)、酸化ストレス(ミエロペルオキシダーゼ、酸化LDL-Cなど)、血管石灰化関連因子（fetuin-A, osteoprotegerin,FGF23）および血管内皮前駆細胞の動脈硬化進展（経時的頸動脈内膜中膜肥厚の変化、プラーク形成）への影響を検討する。また、併用薬との関係を検証する。

The purpose of the study is to assess influence of classical risk factors of atherosclerosis(hypertension, lipid abnormality etc), inflammation(interleulin-6, high sensitive C-reactive protein), oxidative stress(ox LDL-C, myeloperoxidase), vascular calcification associated factors(fetuin-A, osteoprotegerin,FGF23), endothelial progenitor cell etc. on atherosclerosis in MHD patients. Moreover influence of medication on the progress of atherosclerosis.
