変性疾患により小脳失調を呈する患者に対する短期集中型の入院リハビリテーション(リハ)が、患者の小脳失調や日常生活動作（ADL）能力に改善に有効であるかを検証する

選択基準/Key inclusion criteria

日本語
小脳失調を徴候とする脊髄小脳変性症患者(SCA6,16qADCAおよびLCCA)であること
認知機能障害がないこと（Mini-mental status examination: MMSE 24点以上）
介助者が1人以下の状態で歩行できること（SARA: Scale for the assessment and rating of ataxiaで点数が6以下: 2本杖や歩行器あるいは介助者とともに10 m以上歩行できる）

Patients with spinocerebellar degeneration with isolated cerebellar symptoms (SCA6,16qADC or LCCA)
No cognitive impairment (MMSE>=24)
Ambulatory or ambulatory with assistance of one person

除外基準/Key exclusion criteria

日本語
以前の脳卒中の後遺症、関節炎による歩行時の疼痛、少しの運動で呼吸困難や狭心痛が発生する、心肺その他の疾患のために持久力が著しく低く、歩行困難な場合
さらに、無作為化割り当ての際に、認知症の既往や他の神経筋疾患、重症感染症、血栓性静脈炎、起立性低血圧、褥瘡、うっ血性心不全、慢性閉塞性肺疾患、臓器移植、最近の手術（冠動脈バイパス術を含む）、荷重時の疼痛などの理学療法を制限しなければならない疾患がある場合

主な結果/Results

日本語
脊髄小脳変性症に対する集中リハ（2時間x4週間）は、小脳性運動失調、歩行、日常生活動作を改善するが、３カ月以降に効果は次第に減弱する。6カ月後に約半数の患者でベースラインより機能は良好であった。リハ後の機能改善の保持は小脳失調の程度に関連することが示唆された。

Short-term benefit of intensive rehabilitation was evident in patients with degenerative cerebellar diseases. Although the functional status tended to decline to the baseline level within 24 weeks, saving of the functional gains was seen in more than half of the patients at 24 weeks after the intervention.
