目的1/Narrative objectives1 腹腔鏡補助下結腸切除術を受ける患者を対象として、遺伝子多型(SNP)が、麻薬の鎮痛効果や副作用発生に与える影響を検討する全ゲノム相関研究(genome-wide association study：GWAS)を実施する To investigate relationships between genomic polymorphisms and analgesic as well as adverse effects of opioids in patients undergoing laparoscopic colectomy

主要アウトカム評価項目/Primary outcomes 電気刺激試験によるフェンタニルの鎮痛効果の定量、術中のレミフェンタニル必要量(時間平均投与量)、手術終了後2,4,6,12,24時間後の、患者自己管理鎮痛法(Patient-controlled analgesia: PCA)による術後フェンタニルの必要量、疼痛スコア、および麻薬の副作用(呼吸数、鎮静スコア、嘔気スコア、嘔吐数)、遺伝子多型網羅解析 analgesic effects of fentanyl eveluated by pain test, amount of remifentanil required intraoperatively, amount of fentanyl required posoperatively, postoperative pain scale, adverse effects of fentanyl, genome-wide analysis of genomic polymorphisms

主な結果/Results 腹腔鏡下手術を受けた350症例でゲノムワイド関連解析(GWAS)を終了し、術後フェンタニルの必要量に高度に影響する複数の一塩基多型を同定した。 We have identified multiple single nucleotide polymorhisms that can srtrongly affect postoperative fentanyl requirements by performing genome-wide association study in 350 patients who underwent laparoscopic colectomy.
