目的1/Narrative objectives1

日本語
植え込み型定常流LVAS（Dura Heart等）を新規に装着したり、もしくは本邦で唯一保健認可のおりているToyobo LVASを装着し、長期入院を余儀なくされている患者に対して、合併症の少ないDura Heart等の植え込み型定常流LVASへ入れ替し、心臓移植までＱＯＬを保ちながら、自宅にて待機することが可能であるかを臨床的に検討することである。

We will reimplant extra-corponeal left ventricle assit devise to implantable left ventricle assist devise in heart failure patiets who must stay in hospital for a long time. We will analyse whether patients who received implantable left ventricle assisit devise could wait for heart transplantation at home with better quality of life.

Toyobo LVASより植え込み型定常流LVASに入れ替えた患者の、生存曲線、合併症の発症を検討する。また心機能の変化、回復の変化を観察する。

選択基準/Key inclusion criteria

日本語
（ア）拡張型心筋症により、高度心不全状態が持続し、最大限の内科的薬物療法や外科手術によっても効果が得られないＮＹＨＡ４度の症例。
（イ）年齢15歳以上75歳未満であること。
（ウ）インフォームドコンセントが本人より得られていること。

(a) Severe heart failure patient whosesymptoms is NYHA 4 after treatment of maximum medication and surgical procedure.
(b) Applicable age is over 15 years old and under 75 years old
(c) Informed concent

除外基準/Key exclusion criteria

（ウ）その他、責任医師が不適当と判断した患者

左室補助人工心臓の入れ替えにより、現在のところ安全に治療が行われており、患者の一部は自宅にて心臓移植を待機している。
