目的1/Narrative objectives1

日本語
消化管ホルモンであるインクレチンは、膵B細胞量を増加させ、インスリン分泌を促進する作用を有する。主なインクレチンホルモンとして下部小腸から分泌されるGLP-1(glucagon-like peptide-1)と上部小腸から分泌されるGIP(glucose-dependent insulinotropic polypeptide)が知られている。現在2型糖尿病治療薬として生体内でGLP-1不活性化を阻害するDPP-4(dipeptidyl peptidase-4)阻害薬がすでに臨床の場で使用されている。一方、膵頭十二指腸切除術、膵体尾部切除術後は膵実質量が減少し、膵B細胞が減少することにより長期的には耐糖能異常を惹き起こすが、その対応策はこれまで講じられてこなかった。今回、膵切除術後にDPP-4阻害薬であるシタグリプチン(ジャヌビア)を投与することにより、膵切除術後糖代謝の改善が期待できるものと考え、膵切除術後の糖代謝、特に膵B細胞機能とGLP-1に注目した検討を行うことを目的とした。

選択基準/Key inclusion criteria

日本語
１）外科手術の適応となる患者。
２）術前よりインスリン療法や経口血糖降下剤を必要としていない患者。
３）20歳以上の患者で被験者本人への説明文書を用いた説明を行い、本人からの文書による同意が得られていること。

1) The patients who have indications of operation.
2) The patients who have no inslin therapy or hypoglycemic agents.
3) Over 20-years-old patients.

除外基準/Key exclusion criteria

日本語
１）腫瘍による十二指腸狭窄を来している症例。
２）術後に縫合不全、腹腔内膿瘍などの合併症を生じた症例。
３）本人から臨床試験参加の同意が得られない症例。
４）進行癌患者；DPP-4に腫瘍増殖作用はないと添付文書に明期されているが ,安全性を考慮し転移・再発の可能性がある進行癌は対象外とする。
　　　５）その他、担当医が本研究の対象として不適当と判断した症例。
