目的1/Narrative objectives1

日本語
膵島移植はインスリン依存型糖尿病（一部のインスリン非依存型糖尿病）の根治的内分泌補填療法として特化した組織移植療法である。移植手技自体は透視下に肝内の門脈穿刺を行い、膵島を点滴として門脈注入するといった比較的安全な手技であり、むしろ膵摘出後の膵消化、膵島分離・精製が煩雑かつ難易度が高い。既に施行されている膵島移植に倣い、安全かつ迅速に膵島を精製、移植し、レシピエントのインシュリン完全離脱を最終目標とし、膵島移植を前提とした摘出膵からの膵島分離における安全性、治療効果判定を行う。

・絶対的インシュリン必要量の減少（移植前に比べての減少もしくは完全離脱）
　・血糖の安定（特に頻発する無自覚の低血糖発作がないこと）

膵島移植後は頻回の血液検査を行う。現在のところ免疫抑制は一生涯行う必要があり、拒絶（血糖上昇、定期的インシュリン投与が必要）の監視も必要である。インシュリン離脱の評価時期（インシュリン要求量、血糖値、C-ペプチド）は移植後1，3，6ヶ月後および1年後、3年後、5年後に行い、記録保管する。

選択基準/Key inclusion criteria

日本語
１、内因性インスリンが著しく低下し、インスリン治療を必要とする
２、糖尿病専門医の治療努力によっても血糖コントロールが困難
３、原則として75歳以下
４、膵臓移植、膵島移植につき説明し、膵島移植に関して、本人、家族、主治医の同意が得られている
５、発症後5年が経過している

1. the necessity of insulin treatment because of the notable decrease of endogenous insulin
2. difficulty of blood glucose controlin spite of diabetes treatment
3. the age below 75 years old
4. enough informed concent about pancreas and islet transplantation including their family
5. the diabetes for more than 5 years

除外基準/Key exclusion criteria

日本語
a)重度の心疾患、肝疾患（心移植または肝移植と同時に行う場合には考慮する）
b)アルコール中毒
c)悪性腫瘍（5年以内に既往のないこと）
d)重症肥満
e)未処置の網膜症
f)その他移植に適さぬもの
