北里大学病院循環器内科に入院・通院あるいは北里大学東病院循環器内科に通院中の患者のうち、医学的に包括的内部障害リハビリテーションの介入が必要とされる患者（具体的には、高血圧、脂質異常症、糖代謝異常、肥満、喫煙、冠危険因子を１つ以上もつ患者を指す）

High-risk patients for cardiovascular disease in Kitasato University Hospital and Kitasato University East hospital who have more than one coronary risk factor and need to have rehabilitation ofinternal dysorders and medical educational training (e.g. hypertenison, dyslipidemia, fasting high glucose/diabetes mellitus, obesity, current smoker)

除外基準/Key exclusion criteria

日本語
運動指導が適さない中等度以上の整形外科的疾患、中枢神経系疾患
重症心不全 (BNP > 200 pg/mL)

having difficulty with the walking due to of leg dysability or central nervous system diseases
severe heart failure (BNP > 200 pg/mL)
