目的/Objectives
目的1/Narrative objectives1	網膜疾患である糖尿病網膜症、網膜静脈閉塞症（中心・分枝）、網膜剥離、脈絡膜疾患である眼サルコイドーシス、急性網膜壊死、悪性リンパ腫などの手術時に採取される網膜組織は、組織切片を作成することによりサルコイドーシスの原因と考えられる細菌感染の有無、非乾酪性肉芽腫、また糖尿病網膜症に罹患している網膜血管の組織構造などを組織切片により確認することが可能となり病態解明の糸口になる可能性がある。硝子体サンプルは感染症の有無をPCR法により確認する。また、VEGFやTNF-αなどの炎症性サイトカイン発現などをELISA法やprotein arrayなどにより確認する。硝子体採取方法は硝子体手術に組織を採取するものであり、２３Ｇ針または硝子体カッターにより患部硝子体を切除し採取する。網膜、脈絡膜、強膜については周辺部の患部網膜を硝子体剪刀で切除し、切除部周囲は網膜剥離予防のため網膜光凝固を行う。組織を採取することにより現在まで解明できていない網膜剥離や糖尿病網膜症に続いて起こり失明原因になる増殖型硝子体網膜症や網膜血管形態、脈絡膜疾患の原因因子を解明出来る可能性がある。また原因解明によりこれらの疾患に対して今後の新しい治療へ応用できる可能性がある。	Tissue from horioretinopathy(include uveitis), diabetic retinopathy, scleritis, retinal detachment, retinal vein occlusion, malignant lymphoma, epiretinal membrane were collected during operation, which should become the useful tool for curing the disease. Collected samples were tested by several basic analysis such as real time PCR, ELISA, western blotting and tissue specimen. The sample of vitreous fluid were collected by vitrectomy with using 23 gage or 25 gage needles. The other tissue were collected by scissors. After that, laser treatment apply for spots of dissected area. We investigate the new findings and treatment for the disease that dose not have any effective treatment.

評価/Assessment
主要アウトカム評価項目/Primary outcomes	薬剤や新しい治療法を開発するのではないため評価することは困難である。組織をPCR、ELISA、組織切片などで疾患の病態を把握することは可能である。	We would not develop the drugs and new treatment method. So, we could not asses from this clinical test. We could asses and hold the clinical condition from the tissue that was collected during operation.

選択基準/Key inclusion criteria	網脈絡膜疾患を有し、硝子体混濁、網膜剥離、網膜増殖膜変化のため視力低下をきたしており手術が必要な患者	The patients who suffered from vitreous opacification, retinal detach, the proliferative change of retina need the operation to cure those disease.

主な結果/Results	現在、150例中87症例を登録し、サルコイドーシスや悪性リンパ腫にたいして硝子体解析が有効であることが判明した。	We performed 87 subject of 150 scheduled subjects at Tohoku University Hospital. In our examination, we noticed that sarcoidosis and lymphoma had signature T cell and B cell population in uveitis.

その他関連情報/Other related information	患者からランダムに採取した組織を使用し、組織切片の作成、PCRによる感染症の有無、フローサイトメトリーによる細胞解析により病因との関連性を前向き研究として行う。	The tissue collected from patients randomly and prepare for histological section, to detect the infection with using PCR, analyze the cell population. This test perform as forward looking research to detect the relationship between the results and pathogenesis.
