目的/Objectives

目的1/Narrative objectives1

日本語
中四国地域の岡山大学関連病院のCVDハイリスク症例を含むCKD患者を対象に前向きに追跡し, CKD進行, 新規透析導入, CVD合併/再発, 死亡等について, そして予測因子や危険因子について検討する。「Ischemia OKAYAMA」のCKD該当症例（CVDハイリスク症例）についても前向き追跡を行う。その結果, CKD進行, 新規透析導入, CVD合併, 総死亡を抑制する上で有用なCKD・CVD診療システムを構築することを目的とする。

The aim of the current study is to prospective follow patients with CKD including high risk patients for CVD in associated hospitals of Okayama University Hospital, and to evaluate risk factors and predictive factors for progression of CKD, ESRD, CVD onset and death.

主な結果/Results

日本語
全8施設における外来CKD患者184名（循環器内科70人, 腎臓内科114人）を解析し複合腎エンドポイント(R-E)および心血管イベント(CVD-E)発症例について検討した。R-E発症42人(51/1,000・年)、CVD-E発症27人(32/1,000・年)であった。R-EおよびCVD-Eの両方を発症したのは7人。CVD既往のある患者では、R-E発症19人(循10人腎9人)、CVD-E発症16人(循9人腎7人)であった。2科を比べるとR-E発症は腎臓内科で高い傾向にあったが、CVD-E発症はほとんど差を認めなかった(それぞれLog-rank p=0.08, 0.97)。5年間のイベント発症率はCRIC studyと比べて良好であった。
