主な結果/Results

僧帽弁輪の高さは、LCX position、RCA positionでphysiologic positionと比較して、それぞれ有意に低かった。僧帽弁輪の前後径、交連間径、周囲長、面積は 3つの心臓の位置間において有意な差を認めなかった。
LCX positionにおける僧帽弁輪の高さの変化率はMR悪化なし群と比較して、MR悪化群で有意に大きかった。 RCA positionにおける僧帽弁輪の高さの変化率はMR悪化なし群と比較して、MR悪化群で有意に大きかった。

OPCABが予定された定期手術患者を対象とする。麻酔導入後、血行動態が落ち着いた時点で3Dzoomモードを用いて僧帽弁と大動脈弁を描出し3Dデータを取得する。心臓脱転後、バイパス術施行中に血行動態が落ち着いた時点で同様の方法で再度3Dデータを取得する。評価項目は僧帽弁輪の横径、前後径、周囲長、面積、高さにAnnulus Height index(height/perimeter×100)を加えた6項目とする。データの解析はoff lineでQLABを用いて行う。

Three-dimensional TEE data of the mitral valve were obtained prior to skin incision and after the heart was positioned for revascularization of the coronary artery.
Following parameters of the mitral annular structure were measured using plug-in software; bicommissural diameter, anteroposterior diameter, perimeter, annular area, annular height, annular height index.
