目的1/Narrative objectives1

日本語
全身麻酔の深度を測定する方法としてbispectral index (BIS)が用いられる。BISは前額部に貼付した電極より得られる脳波のバイスペクトラル解析によって得られ、完全覚醒を100、平坦脳波を0とした0-100の数字で表され、手術中は40-60 が適度な麻酔深度とされる。近年本邦で臨床使用が開始された吸入麻酔薬であるデスフルランは、従来の吸入麻酔薬に比べて導入・覚醒が速く、高齢者や長時間の麻酔の際には極めて有用であるとされている。しかし加齢に伴うデスフルランの麻酔深度の変化、すなわちMACBIS50の変化ついては明らかになっていない。そこで全身麻酔下で手術を受ける若齢者(30歳以下)、中齢者(31-65歳)、高齢者(66歳以上)を対象にMACBISを測定し、年齢間におけるMACBIS50の変化を検討する。

Desflurane is a new inhalational anesthetic with rapid onset and withdrawal, which is useful for general anesthesia in elderly patients. However, its effect on depth of anesthesia in different age groups has not been elucidated. Bispectral index is useful for measuring depth of general anesthesia. We compare the minimum alveolar concentration of desflurane for maintaining bispectral index below 50 in 3 different age groups.

気管挿管下の全身麻酔で手術を受ける、American Society of Anesthesiologists (ASA) クラスIまたはII の症例。

Patients undergoing general anesthesia under tracheal intubation with American Society of Anesthesiologists physical status I or II

除外基準/Key exclusion criteria

１）喘息　２）不整脈　３）精神疾患、神経・筋疾患　４）虚血性心疾患および駆出率40%以下の低心機能　５）収縮期血圧が80 mmHg以下の低血圧　６）電解質異常

Patients with bronchial asthma, arrhythmia, psychiatric disorder, ischemic heart diseases, low cardiac output with ejection fraction equal to or less than 40%, systolic blood pressure below 80 mmHg or abnormal serum electrolytes
