日本語
筋萎縮性側索硬化症と診断され、以下の基準をすべて満足する患者を対象とする。
1)EL Escorial 改定 Airlie House 診断のうち、「definite」、「probable」、「probable-laboratory-supported」「possible」「suspected」のいずれかに適合する患者で、ALS重症度分類における重症度分類1~3度の患者
2)年齢が20歳以上、70歳未満である患者
3)本研究に参加することへの同意を文書で得られた患者
疾患対照は、以下の基準をすべて満足する患者を対象とする。
1)年齢が20歳以上、75歳未満であるPDおよびMS患者
2)神経内科外来を受診した疾患対照者に研究協力医師が説明し参加をお願いして同意が得られた患者
3)本研究に参加することへの同意を文書で得られた患者
健常人の場合は、以下の基準をすべて満足する健常人を対象とする。
1)年齢が20歳以上、65歳未満である健常人
2)臨床研究パンフレットで募集し興味を持たれた方
3)本研究に参加することへの同意を文書で得られた健常人

Patients diagnosed as having ALS and satisfying all of the following criteria.
1)ALS patients who are diagnosed by EL Escorial revised Airlie House criteria as clinically definite,clinically probable or clinically probable-laboratory-supported, clinically pssible ALS or clinically suspected ALS, and Stage I, II or III in ALS severity classification.
2)Older than 20 and younger than 70 years old
3)Patients who give informed consent after detailed explanation of the study
As disease controls, PD or MS patients satisfying all of the following criteria.
1)Older than 20 and younger than 75 years old
2)Patients who consulted a nerve outpatient and obtained consent for study entry
3)Patients who give informed consent after detailed explanation of the study
Healthy individuals satisfying all of the following criteria.
1)Older than 20 and younger than 65 years old
2)Healthy individuals who have interest in this study
3)Healthy individuals who give informed consent after detailed explanation of the study

1)病名の未告知の患者
2)重度の精神症状(幻覚、妄想)や認知症を有する患者
3)重篤な起立性低血圧や低血圧症を有する患者
4)重篤な心疾患、腎疾患、肝疾患などの合併症を有する患者
5)好酸球値が上がりやすいアレルギー疾患の合併症を有する患者
6)同意能力を欠く患者
7)その他、担当医師が本試験への参加において不適切と判断した患者
8)同意取得3ヶ月以内に他の臨床治験または臨床試験に参加した患者
9)他の臨床治験に参加している患者
*既存の治療薬の投与患者は除外基準に該当しないとする。

2)Patient having severe psychiatric symptoms (hallucination, delusion) or dementia
3)Patient having serious orthostatic hypotension or other types of hypotension
4)Patient having a severe complication(s) such as heart, kidney and/or liver disease
5)Patient having the complications of the allergic disease that an acidophile level is easy to rise
6)Patient did not have ability to comprehend informed consent
7)Patient who was inappropriate for this study
8)Patient who has received an investigational drugs or devices within three months before
9) Patient who participates in other clinical trials
*The patient treated with existing therapeutic drug does not correspond to exclusion criteria

結果 3コースの開始から終了までの平均期間は319.7±33.7日であった。2分間歩行テストと10m歩行テストについて、ベースライン値、各参加者のID、時点を共変量として重回帰分析を行った。各コース後の変化を転帰とみなした。3つの治療コースの後、2分間歩行距離はベースライン値と比較して16.61m(95%信頼区間、-9.33~42.54)改善したが、この改善は統計的に有意ではなかった(p=0.21)。しかし、ケイデンスは1.30歩(95%信頼区間、0.17-2.42;p=0.02)有意に改善した。結論 長期間の反復HAL治療はALS患者の歩容維持に役立つ可能性がある。

The average time from the start to the end of the 3 courses was 319.7(33.7) days. A multiple regression analysis was performed for the 2-minutes-walk and 10-m-walk tests, using the baseline value, each participant's ID, and time point as covariates.

Methods Three courses of treatment with HAL were administered to three women with ALS. Each course had a four- to five-week duration, during which the treatment was performed nine times, with a rest period of at least two months between each course. Gait ability (2-minutes-walk and 10-m-walk tests), ALS Functional Rating Scale-Revised, and respiratory function tests were performed before and after each treatment course. Patients Patients diagnosed with ALS, according to the updated Awaji criteria, by board-certified neurologists in the Department of Neurology and Department of Rehabilitation Medicine, Toho University Omori Faculty of Medicine between January and December 2019 were recruited.
