日本におけるMIRCERA発売年月である2011年7月以降にMIRCERA若しくはその他ESAを継続して少なくとも6ヵ月間(初回投与日と最終投与日の日数差が180日)以上投与した慢性腎不全患者を対象とする。

The analysis set will include patients with chronic renal insufficiency who meet these conditions and have continuously receive MIRCERA or other ESAs for a duration of at least 6 months (the difference between the date of initial dose and the date of final dose is at least 180 days) since July 2011 (after the product launch of MIRCERA) or later.

背景:
Real Word設定におけるベネフィット・リスク(B-R)の評価は、医薬品のための重要な課題であるが、いくつかの研究により、このアプローチを探求してきました。ベネフィット・リスク・アクションチーム(BRAT)フレームワークを適用することにより、医療データベースを使用して、B-R評価の有用性と限界を調べるために、我々は、エリスロポエチンに関してケーススタディを行いました。
方法:
Medical Data Visionの医療データベースから市販後のデータを用いて、メトキシ・ポリエチレングリコール - エポエチンベータ(C.E.R.A.:持続性エリスロポエチン受容体活性化薬),その他の赤血球造血刺激剤(ESA)とでB-R比較を行いました。データは2011年7月から2014年3月までのC.E.R.A.で治療中の慢性腎臓病(CKD)患者から131例(非透析109例,血液透析22例),他のESAで治療中の 542例(非透析327例,血液透析215例)を得ました。
結果:
C.E.R.A.のB-Rプロファイルは主にオッズ比で評価した。非透析,血液透析の両CKD患者における他のESAのB-Rプロファイルと同様であるように思われました。各アウトカムに対する様々な点推定値と信頼区間にもかかわらず、サブグループの結果は、B-R評価の全体的な分析と比較して顕著な差異を示さなかった。
結論:
B-Rの評価は、医療データベースとBRATフレームワークを使用して行うことができるが、単一のデータソースを使用する場合には制限が存在します。データ抽出の際の選択,アウトカムを定義するときには注意が必要です。さらなる研究により,このアプローチの実用化を促進することが必要です。

Background
While benefit-risk (B-R) assessment in the real-world setting is an important challenge for pharmacovigilance, few studies have explored this approach. To investigate the utility and limitations of B-R assessment using a healthcare database by applying the Benefit Risk Action Team (BRAT) framework, we have conducted a case study with erythropoietin agents.
Methods
Post-marketing data from the Medical Data Vision healthcare database were used in a B-R comparison between methoxy polyethylene glycol-epoetin beta (continuous erythropoietin receptor activator: C.E.R.A.) and other erythropoiesis stimulating agents (ESAs). Data were from patients with chronic kidney disease (CKD) treated with C.E.R.A. (n=131: nondialysis, n=109; hemodialysis, n= 22) or other ESAs (n=542: nondialysis, n= 327; hemodialysis, n=215) between July 2011 and March 2014.
Results
The B-R profile for C.E.R.A. appeared to be similar to that for other ESAs in both nondialysis and hemodialysis patients with CKD, when benefits and risks were mainly assessed in terms of odds ratios. Despite various point estimates and confidence intervals for each outcome, the results of subgroup analyses showed no notable differences from the overall analysis in B-R assessment.
Conclusions
B-R assessment can be performed using the BRAT framework with a healthcare database, but limitations exist when using a single data source. Care should be taken when selecting data for extraction and defining outcomes of interest. Further research is necessary to facilitate practical application of this approach.

本疫学研究の目的及びB/Rの分析を行う上で,必要なデータが揃っており,分析に最適と考えられるMDV(Medical Data Vision:メディカル・データ・ビジョン)社の診療DBを用いる。MDV診療DBは病院から匿名化された症例情報を集めたDBで,電子レセプトとDPCファイルを基に契約している病院にて匿名化されたものをMDV社にてDB化したもので,2014年5月現在,日本国内の急性期病院(DPC参加病院)147施設約750万人の患者情報が集積されている。対象とする患者数や処方日数,処方量等の分析が可能なツールであり,疾患や薬剤だけでなく,手術や検査など実際の診療行為を起点とした多角的な分析も可能である。

The MDV medical record database is selected as the most suitable data source for the intended study due to the fact that it contains all necessary data required to achieve the objective of the epidemiological study and to conduct the benefit-risk analysis as planned. The MDV medical record database consists of anonymized patient information collected from hospitals. Data from electronic receipts and Diagnosis Procedure Combination (DPC) files are anonymized at contracting hospitals and sent to MDV for the generation of a database. It contains medical information derived from approximately 7.5 million patients at 147 acute-phase medical institutions participating in the DPC system in Japan as of May 2014. It provides a tool that allows not only the analysis of the number of patients and the amount of drugs prescribed in a specific field of interest, but also a multidimensional analysis based on actual medical treatments, including surgery and examination.
