日本語
睡眠時無呼吸症候群（SAS）は、虚血性心疾患、高血圧、心不全といった種々の循環器疾患の増悪に寄与する要因の一つであることが知られており、我が国における有病率は欧米に比較しても低くはない。特に虚血性心疾患に関しては、SASの重症度は、脂質異常症、耐糖能異常などの冠危険因子とは、独立した因子として発症に相関するとの報告もある。
　また、筋交感神経活動(MSNA)を直接記録することで、SASで全身交感神経系が活性化されていることが証明されており、SASの重症度との関連について、多くの検討が行われている。
　本研究はSAS患者に対して施行した、冠動脈CT検査、及びMSNAの直接記録を用いて、SAS患者における全身交感神経活性と、虚血性心疾患発症のリスクの関連を評価することを目的とする。

Sleep apnea syndrome (SAS) is known as a risk factor of cardio vascular diseases. As to coronary artery disease, SAS is said to be one of the independent risk factor. On the other hand, augmented sympathetic nerve activity in SAS patients was associated with worsening disease. And sympathetic nerve activity is able to be measured by direct recording of muscle sympathetic nerve activity(MSNA). So we performe a retrospective study to elucidate the correlation between single and multiunit MSNA and apnea hypopnea index (AHI) in SAS patients. Subsequently, we try to clarify the correlation of this results and prevalence of coronary artery diseases.

すでに記録されている、睡眠時無呼吸症候群患者の、筋交感神経活動、終夜睡眠ポリソムノグラフィー、冠動脈CTの結果を用い、睡眠時無呼吸症候群の重症度と、全身の交感神経の緊張度、冠動脈疾患の合併率の関係を評価し、睡眠時無呼吸症候群患者の冠動脈病変の合併を防ぐための治療の指標を明らかにすることを目的とする。

We assume relationship between the severity of sleep apnea syndrome(SAS), sympathetic nerve activity and prevalence of coronary artery disease, using muscle sympathetic nerve activity, full polysomnography and coronary CT, which were already recorded.
The purpose is to crarify the therapeutic target of SAS patient in order to avoid coronary artery disease.

除外基準/Key exclusion criteria

1. 冠動脈CT施行前に狭心症、心筋梗塞により、血行再建術を施行した既往がある
2. 心機能低下例(左室駆出率＜55％）
3. 心不全
4. 高度腎機能低下(推定糸球体濾過率<30ml/min/1.73m2)
5. 診療データの利用に関し拒否を申し出た患者
