TKAにおいて脛骨回旋の参照となる脛骨正面軸が、(1)検者内、検者間で差があるか、(2) CTベースナビゲーションで登録した脛骨正面軸とマニュアルで決定した脛骨正面軸とどの程度差があるか、を前向きに調査し、その成績を比較することが本研究の目的である。

CTベースナビゲーションを用いて脛骨正面軸と決定したのち、2人の検者で脛骨骨切り前と後に、それぞれ2回ずつmanualで脛骨正面軸を計測すること。

After registration of the rotational position of tibial component using the CT-based navigation system, the position was compared with the position which was registered by the manual method. The manual registration is performed by two surgeons in duplicate before and after proximal tibial cut.

変形性膝関節症、膝骨壊死などの膝関節疾患で、当院にてTKAを施行する男女30症例を、年齢を問わず対象とする。

CT-Akagi’s line とM-Akagi’s lineとの角度差は検者A / Bで骨切り前2.5±5.2°/ -0.1±5.5°、骨切り後1.1±6.2°/ -3.1±6.1°であった。Outlier発生率は検者A / Bで骨切り前23% / 18%、骨切り後18% / 23%であった。

2015年3月から2016年7月までに変形性膝関節症でTKAを施行した22例22関節を対象に前向き研究を行った。男性4例、女性18例であり手術時の平均年齢は75歳 (57 - 87歳)であった。

術前に撮影した下肢全長CTをOrthomap3D に取り込み、横断像を参考に膝蓋靱帯付着部内側縁とPCL付着部中央を同定し、2点を結ぶ線をCT-Akagi’s lineと定義した。 CT based navigation system (OrthoMap 3D Navigation System, Stryker)を用い、CTとナビゲーションをmatchingさせた後、ナビゲーションガイド下、CT-Akagi’s lineにK-wireを刺入した。ナビゲーションをCT-free navigation system (precisioN Knee Navigation System Ver.4.0, Stryker)に切り換え、大腿骨頭中心、膝関節、足関節中心等、各解剖学的基準点の登録を行った。この際、K-wireを刺入したCT- Akagi’s lineを脛骨前後軸として登録し、登録後にK-wireは抜去した。検者はナビゲーションの画面を見ることなしに、膝蓋靭帯内側縁とPCLをメルクマールとして、目視でAkagi’s lineを登録した。このラインをM-Akagi’s lineと定義した。計測は2人の検者が、脛骨の骨切り前と後に、それぞれ2回ずつ行った。CT-Akagi’s line を基準軸としてM-Akagi’s lineとの角度差をナビゲーションで計測した。基準軸に対して内旋設置をプラス、外旋設置をマイナスで表記した。また、角度差が7°以上をoutlier6)としたときのoutlier発生率について調査した。

Preoperative computed tomography scans were performed to identify the Akagis line. The line was incorporated in the CT based navigation system and defined as CT-Akagis line. Two observers (observer A / B) determined the Akagis line (M-Akagis line) manually without referring navigation before and after proximal tibial cut. The angle between CT-Akagis line and M-Akagis line and the outlier of the angle were investigated.
