【背景】「新規効能取得のための抗シワ製品評価ガイドライン」に附則する「シワ写真撮影ガイドライン」は目尻のシワを対象にした定性評価法であり、顔面マッサージによる小顔効果を定量的に評価することはできない。また、その煩雑さにより、小規模サロンにおける日常業務での実行可能性は低い。
【目的】顔面マッサージの定量評価を目的とした簡易型顔面画像評価法の開発と実行可能性の検討
【方法】
研究デザイン：前向きコホート研究
対象：2014年11月12日～18日に我々の施設で顔面マッサージを受けた8症例（女性8例、年齢46±6.8）。
測定方法：顔面マッサージ施術前後に顔面を規定の撮影方法でデジタルカメラ撮影。床マーカーに撮像椅子脚およびカメラ三脚を合わせることにより、撮像椅子とカメラ三脚の位置の再現性を担保した。背景壁に眼位マーカーを設置し、後頭部を壁に接することで、頭位の再現性を担保した。臨床情報を知らされていない画像診断専門医1名によって、術前後の画像を定量評価した。
評価方法：術前後の撮影再現性を、外眼角間距離、内眼角間距離で評価した。下顎の引き上げ効果を、内眼角と下顎との垂直距離で評価した。口角の引き上げ効果を、内眼角と口角の距離で評価した。
統計解析：統計学的解析はpaired t testを使用し、P<0.05を統計学的有意とした。
【結果】
8症例16画像中16画像（100%）で定量評価が可能であった。
撮影再現性は、外眼角間距離(P=0.67)、内眼角間距離(P=0.50)ともに術前後に有意な差を認めなかった。
下顎の引き上げ効果は、術後に有意に内眼角と下顎の垂直距離が減少していた(P=0.04)。
口角の引き上げ効果は、術前後で有意な差を認めなかった(P=0.21)。
【結語】
顔面マッサージの定量評価を目的とした簡易型顔面画像評価法を開発した。
本法は実行可能であり、顔面マッサージ効果を定量的に評価できた。
