主要アウトカム評価項目/Primary outcomes

バルーン補助下化学塞栓療法の安全性について化学塞栓療法の副作用を除く有害事象について、SIR(Society of interventional radiology) standards of practice committee classification of complications by outcomeに従って評価する。

To evaluate the safety of BACE, adverse events except for the side effects of chemoembolization are assessed according to SIR(Society of interventional radiology) standards of practice committee classification of complications by outcome.

通常の化学塞栓療法では正常肝実質の治療範囲が大きいと考えられる症例に対して、microballoon catheterで正常肝実質に向かう動脈を一時的に閉塞し,2本目のmicrocatehterより化学塞栓療法を行う。

In cases where a large volume of normal hepatic parenchyma is expected to be damaged by ordinary chemoembolization, a microballoon catheter is used to temporarily occlude the artery perfusing the normal hepatic parencyma and chemoembolization is performed through another microcatheter.

選択基準/Key inclusion criteria

日本語
1)EASLガイドラインで肝細胞癌と診断されている。2)化学塞栓療法を依頼されている。
3)Performance statusが0-2
4)肝機能がChild-Pugh分類のAまたはB
5)eGFRが30mL/min/1.73m2以上
6)血小板が50000/μL以上
7)前治療がないか、あっても4週間以上が経過している
8)患者本人の同意

英語
1)Diagnosis of hepatocellular carcinoma according to EASL guideline
2)Chemoembolization is indicated.
3)Performance status of 0-2
4)Liver function of Child-pugh A or B
5)eGFR is 30mL/min/1.73m2 or more
6)Platelet count of 50,000 microL or more
7)No previous treatment or 4 weeks after the previous treatment
8)Informed consent is obtained.

除外基準/Key exclusion criteria

日本語
1)肝細胞癌破裂後
2)外科的胆道再建や内視鏡的胆管治療の既往
3)高度の動脈-門脈シャントや動脈-静脈シャント
4)腹腔動脈や肝動脈に解離や50％以上の狭窄
5)肝性脳症や重度の精神障害
6)ヨード造影剤やその他の薬剤の重度のアレルギー
7)その他医師が本試験を安全に実施するのに不適当と判断する

1)Rupture of hepatocellular carcinoma
2)History of surgical or endoscopic treatment of bile duct
3)Prominent arterioportal or arteriovenous shunts
4)Dissection or stenosis of 50% or more in the celiac trunk or hepatic artery
5)Hepatic coma or severe psychiatric disorder
6)Severe drug allergy including iodine contast allergy
7)Inadequate patients to undergo the procedure safely
