目的/Objectives

日本語
1. 国立国際医療研究センター病院・トラベルクリニック(DCC)を受診し、皮膚症状を主訴のひとつ、あるいは、皮膚症状が病態と密接に関連する疾患を有する患者の診断、転帰、その他の状況等の実態把握を行う。
2. Teledermatologyをスムーズかつ効果的に運用する1)専用アプリケーションソフトウェア、2) 専用解析ソフトウェア開発を実施する。
3. スマートフォン撮影画像、デジタルカメラ撮影画像、対面診療(face-to-face)による熱帯皮膚病の診断率の比較検討および専用アプリ使用の有用性を評価する。
4. 当センター・DCCを受診し、皮膚症状を有する患者でのstore and forward (SAF) 法を用いた遠隔画像診断(teledermatology)システムを構築する。
5. 西アフリカ・コートジボワールでの学童皮膚検診（主任研究者と現地保健省・NGO共同プロジェクト）と当センターを3で構築した遠隔診療で結び、その有用性を検証する。

1. To understand the spectrum of skin diseases in travelers visiting the traveller's clinic of the National Center for Global Health and Medicine (NCGM)
2. To develop a smartphone application software and analysis software which will be applicable to a broad setting including low- and middle-income countries.
3. To assess the efficacy of the smartphone application software as compared to the conventional face-to-face consultation.
4. To establish a tele dermatology system to connect physicians / health workers within and outside NCGM.
5. To apply the tele dermatology system in a school-based skin survey project in Cote d'Ivoire.

国立国際医療研究センター病院のトラベルクリニックを、皮膚症状を主訴のひとつ、あるいは、皮膚症状が病態と密接に関連する疾患にて受診した患者を対象とする。年齢は、問わない。しかし、未成年者の場合には、法的な保護者の同伴があり、保護者からの同意が得られるときのみ対象とする。

Patients with skin symptoms who visited the traveller's clinic of the National Center for Global Health and Medicine will be included in the study. Patients under the age of 18 will need a consent form of a legal guardian.

除外基準/Key exclusion criteria

他院受診あるいは自己判断にて既に治療を開始し、皮膚症状に修飾がある者は、除外とする。

NCGM内Teledermatology: 計23名のリクルートが終了となっている。

NCGM-コートジボワール間Teledermatology:
2015-2021年度にかけ、学童ならびに集団検診にて皮膚疾患の診断を実施。重症患者をTeledermatologyに登録。クリニックにおいては、皮膚疾患と潰瘍患者のTeledermatologyへの登録を実施。

Adzope地域: 計986名の児童の皮膚検診
Gagnoa地域:計4,068名の児童の皮膚検診
Oume地域：計164名の皮膚疾患と潰瘍患者登録

検診データについては、解析終了。Teledermatologyに登録のあった患者は随時新規データの解析を実施。

NCGM Teledermatology: A total of 23 patients have been recruited.

NCGM-Cote d'Ivoire Teledermatology:
During the school and community skin surveys between 2015-2021, patients with severe skin diseases were enrolled to the teledermatology system. At clinics, patients with skin diseases/wounds were enrolled.

Adzope: 986 schoolchildren screened
Gagnoa: 4068 schoolchildren screened
Oume: 164 patients with skin diseases/wounds enrolled and being followed up.
