目的1/Narrative objectives1

日本語
健常者、メタボリック症候群の予備群、および、メタボリック症候群該当者を対象とした断面調査を行う。生活習慣と内臓脂肪、生活習慣と動脈硬化度、内臓脂肪と動脈硬化度との関連を検討する。次に、メタボリック症候群の予備群およびメタボリック症候群該当者が保健指導を受けた場合に、生活習慣が変化するかを明らかにし、効果的な保健指導が内臓脂肪を改善するかを前向きに調査する。内臓脂肪改善の要因として保健指導を受けた後の日常生活における活動量を測定し、内臓脂肪の変化との関連を検討する。さらに、特定保健指導に伴う生活習慣改善と内臓脂肪の改善が、酸化ストレスと動脈硬化に好影響をもたらすかを明らかにする。

A cross-sectional study is planned to elucidate the relationships between lifestyle, visceral fat, and arterial stiffness in subjects who are classified as normal, pre-metabolic or metabolic syndrome on annual special health checkups. Then, observational studies are planned to clarify whether lifestyle modifications supported by regional health nurses should improve visceral fat and arterial stiffness in subjects with pre-metabolic or metabolic syndrome. Especially in those with metabolic syndrome, oxidative stress will be examined before and after lifestyle modifications.

選択基準/Key inclusion criteria

研究対象者の選定方針：2015年から2019年埼玉県の市町村国民健康保険組合が実施する特定健康診査対象者の中で、研究日程に合う時期（年間2～3回）の該当者全員に結果送付時に本試験に関する案内を同封し募集する。応募者が多い場合は先着順とする。
1.健常者
埼玉県の特定健康診査受診者で年齢が40～64歳で、生活習慣と運動習慣に関するアンケート調査、および、特定健康診査の結果を提示することに同意した健常者でメタボリック症候群に関する「情報提供」に階層化された方、年間10名、5年間で50名を対象とする。
2.メタボリック症候群予備群の動機付け支援対象者
埼玉県の特定健康診査受診者で年齢が40～64歳で、生活習慣と運動習慣に関するアンケート調査、および、特定健康診断の結果を提示すること、さらに、特定保健指導の実践に同意したメタボリック症候群の予備群で「動機付け支援」に階層化された方、年間10名、5年間で50名を対象とする。
3.メタボリック症候群の積極的支援対象者
埼玉県の特定健康診査受診者で年齢が40～64歳で、生活習慣と運動習慣に関するアンケート調査、および、特定健康診断の結果を提示すること、さらに、特定保健指導の実践に同意したメタボリック症候群で「積極的支援」に階層化された方、年間10名、5年間で50名を対象とする。
実施場所：埼玉県市町村　保健センター（毛呂山町および飯能市）　今後も協力機関を募る
健常者については断面調査に、メタボリック症候群予備群、メタボ症候群に関しては断面調査とコホート研究に利用する。

糖尿病または高血圧と診断され、かかりつけ医が診療している患者
糖尿病や高血圧で投薬を受けているもの
心血管病の既往のあるもの
急性および慢性炎症疾患にかかっているもの
悪性疾患の治療を受けているもの

本研究では健常者、メタボリック症候群予備群、および、メタボリック症候群該当者を対象とした断面調査を行う。日常生活内での活動量、内臓脂肪、および、動脈硬化度を測定し、生活習慣に関するアンケート調査と特定健康診断の項目との関連を検討する。次に、メタボリック症候群並びに予備群が保健指導を受けた場合に、生活習慣が変化するかを明らかにし、保健指導が内臓脂肪・動脈硬化度および酸化ストレスを改善するかを前向きに調査する。これらの成果は地域の保健予防活動の医学的な貢献を明示するものとなる。

A cross-sectional study is planned to elucidate the relationships between lifestyle, visceral fat, and arterial stiffness in normal subjects, in subjects who were classified as normal, pre-metabolic and metabolic syndrome on annual special health checkups. Then, observational studies are planned to clarify whether lifestyle modifications supported by regional health nurses should improve visceral fat and arterial stiffness in subjects with pre-metabolic or metabolic syndrome. Especially in those with metabolic syndrome, oxidative stress will be examined before and after lifestyle modifications. Daily physical activity will be measured for several days during the 3- to 6- month period with lifestyle modifications in those with pre-metabolic or metabolic syndrome. The potential beneficial effects on cardio-metabolic parameters after lifestyle modifications will encourage the subjects themselves and health nurses to continue their health promotion.
