目的1/Narrative objectives1

日本語
腸内細菌叢はヒトにおいて様々な生物学的側面に影響を及ぼす。腸内フローラの機能・組成の変化は炎症性腸疾患、慢性炎症、脂質異常症、糖尿病、アトピー性疾患、心血管系疾患、癌、肥満と関連している。しかしながら腎機能障害がいかに腸内細菌叢を変化させるか、また、腎機能障害患者における腸内細菌相変化が進行性の動脈硬化、骨代謝異常、全身性慢性炎症などの合併症に影響を与えているかについてはほとんどわかっていない。多発性嚢胞腎（Polycystic kidney disease; PKD）は遺伝性であり、進行性の腎機能障害を伴う。今回我々は進行性、非進行性PKD患者における腸内細菌叢について横断的に検討を行う。また、テストステロンは加齢とともに低下することが明らかとなり、この加齢による変化が加齢男性性腺機能低下症(Late-onset hypogonadism)として筋力低下、内臓脂肪増加、メタボリック症候群の発生、糖尿病・心血管系病変の発症につながる事があきらかとなった。これらと関連して肥満の発症については腸内細菌叢が関与していることが注目されており、両者の関連が前述の理由からも注目されるが、性ホルモン環境と腸内細菌叢の関連についてはまだほとんど明らかではない。今回、我々は前立腺がん患者における男性ホルモン遮断療法前後の腸内細菌叢の変化について縦断的に検討を行う。

Intestinal flora is related to Inflammatory bowel disease, Chronic inflammation, Diabetes, cancer, and so on. However, the relation with chronic kidney disease is unclear. We will test Intestinal flora of progressive ADPKD or non-progressive ADPKD. Furthermore, to evaluate the relation of Intestinal flora and sex hormone, we will test Intestinal flora of prostate cancer patients at before and after the treatment of LH-RH.

順天堂大学医学部附属病院泌尿器科外来通院中の多発性嚢胞腎患者、前立腺がん患者
前立腺がん患者においては新規に診断され、アンドロゲン除去療法が必要と判断されたもの。

除外基準/Key exclusion criteria

日本語
精神科疾患または心理学的異常があるもの
4. 2. 2 自己免疫性疾患の既往があるもの
1:免疫抑制剤、抗菌薬・細菌製剤(継続的使用に限る)を使用しているもの
2:内視鏡治療以外の腸管手術の既往があるもの
3:重篤な消化器系疾患を有するもの。
4:文書での同意が得られなかった者
5:研究責任医師または研究分担医師が対象として不適当と判断した患者
