萎縮性胃炎を有する患者に対して、上部消化管内視鏡検査を施行し、通常の白色光観察に加えて画像強調観察を施行することにより早期胃癌の検出の有効性を検討する。

主な結果/Results

日本語
全体で早期胃癌15例、胃腺腫1例の計16例（3.2%）を認めた。そのうち最初のWLIで検出しえたのは早期胃癌11例、胃腺腫1例の計12例（75%）で、WLIで検出されず、LCIまたはBLI-brightで検出されたのは早期胃癌4例（25%）であった。4例のいずれも1cm以下の発赤調の病変で、IIa 1例、IIc 3例であった。平均病変径は7.8mmとWLIで検出した病変（平均21.2mm）と比べて有意に小さかった。

16 gastric neoplasms (3.2%) were detected. 11 EGCs and one gastric adenoma (75%) were detected by first WLI mode, whereas 4 EGCs (25%) were missed by the first WLI mode and were detected by LCI mode or BLI-bright mode. All of them were less than 1cm in diameter, reddish, one was protruded type and 3 were depressed type. Mean diameter of the lesions was 7.8mm and significantly less than that (21.2mm) of the lesions detected by the first WLI mode.

初めに通常光で観察し、その後、BLI-brightのモードで観察、さらにLCIモードで観察し、病変が疑われれば白色光での観察を再検し、インジゴカルミンを用いた色素内視鏡を施行することにより病変が疑われた場合には生検を施行し、病理組織学的に診断する。
