目的/Objectives
目的1/Narrative objectives1	パーキンソン病患者における手続き学習能力や神経心理検査、脳MRI画像上の脳神経回路の損傷程度とリハビリテーション効果との関連を検討する。	To examine the relevance between the efficacy of the rehabilitation and neuropsycological tests including procedural learning ability or brain MRI image in Parkinson Disease

評価/Assessment
主要アウトカム評価項目/Primary outcomes	評価は入退院時（８週間の入院リハビリテーションプログラム）及び退院後３ヶ月、６ヶ月、１２ヶ月で行う。 UPDRS part 2	The assessments will be done on the time of admission, discharge, 3 months, 6 months, 12months after discharge, respectively. The primary outcome is the UPDRS part 2.
副次アウトカム評価項目/Key secondary outcomes	UPDRS part 3、mini-BESTest、１０m歩行速度	The UPDRS part 3, the mini-BESTest, the 10m walk time.

参加者背景/Baseline Characteristics	健常者　15名 パーキンソン病患者 20名
参加者の流れ/Participant flow	患者群は2016年8月承認日から2019年3月31日の間に兵庫県立リハビリテーション西播磨病院に入院でリハ ビリテーションを行うPD患者のうち前述の基準を満たす者を対象とした。 健常者は兵庫県立リハビリテーション西播磨病院のホームページ上で参加募集を行い、前述の基準を満た す希望者を対象とした(職員は対象外)。 　患者への各種評価、リハビリは全て兵庫県立リハビリテーション西播磨病院で行う。 PD患者に対して入院にて8週間の集学的リハビリ介入を行う。基本のリハビリとして理学療法40分/日 ×6日/週、作業療法40分/日×6日/週、言語療法40分/日×6日/週を行う。さらに、自転車エルゴメー ター、ウォーキング、水中運動といった60分/日×2日/週の有酸素運動や、多重課題である知覚運動やリ ズム運動を用いた認知トレーニングを60分/日×2日/週行う。またペーパートレーニングとして短期記 憶・ワーキングメモリ課題、注意訓練、遂行機能課題。視覚認知課題を行う。 入院中に抗PD薬の調整は行わない。
有害事象/Adverse events	なし
評価項目/Outcome measures	●リハビリの効果の予測因子 ①順序ボタン押し課題 2回 (PD患者、健常者) PD患者と健常者を対象に入院時(初回)と1ヶ月(2回目)時に手続き学習能力として順序ボタン押し課題 を評価する 順序ボタン押し課題はリハビリテーションの時間外で検査の一環として無償で実施する。 ②神経心理検査、非運動症状アンケート1回(PD患者:全検査、健常者:MMSEのみ) ・一般認知(MMSE) ・うつ(GDS) ・アパシー(やる気スケール) ・注意機能・ワーキングメモリ (Digit span、Tapping span) ・前頭葉機能・遂行機能 (WCST、Iowa Gambling Task) ・記憶 (WMS-R 視覚性再生) ・視空間機能 (WAIS積木模様) ・睡眠障害、覚醒障害、疼痛、疲労 ③脳MRI検査 (PD患者、健常者) 拡散テンソル画像で神経回路の損傷程度を評価する。Fractional Anisotropy(異方性比率:FA)画像 による白質線維の質的・量的定量化とVBM(Voxel-based morphometry)画像による脳皮質体積の定量化を 行う。 脳画像検査は入院時(1回)に実施する。PD患者は一般診療とて実施し、健常者は無償で実施する。 ●リハビリの効果(PD患者) 入院時、退院時、退院後3ヶ月、6ヶ月、12ヶ月にADL、運動症状、QOLを評価する。 ・主要評価項目 : UPDRS part2(ADL) ・副次評価項目 : UPDRS part3 (運動)、バランス(Mini-BESTest)、歩行(10m歩行)
個別症例データ共有計画/Plan to share IPD	研究で収集した情報は全て兵庫県立リハビリテーション西播磨病院で保管する。
個別症例データ共有計画の詳細/IPD sharing Plan description	研究で収集した情報はすべて兵庫県立リハビリテーション西播磨病院に保存し、「学校法人兵庫医科大学 における研究活動の不正行為に関する取扱規程」に基づき当該成果の発表から10年間、試料や標本などの有体物は5年間原則保存する。外部と切断されたパソコン端末を使用し、記憶媒体は講座内の鍵のかかるロッ カーで保管する。保存期間後は、電子データは消去し、その他については廃棄する。
