本研究では、本邦での門脈合併PDにおける左側門亢症に対する対策とその実情を把握するとともに、その中から最も適切な手術方法を導き出す。

門脈合併切除、脾静脈切除を施行した際、左側門亢症を防ぐために、各施設で様々な工夫がなされているが、その中で最も適切な手術方法を他施設のアンケート調査にて導き出す。

To identify it, we evaluate the incidence of gastrointestinal bleedings from the varices, platelets counts and spleen volume on POD 6 and 12 months

三重大学肝胆膵外科にて２００５年から２０１５年にかけて施行された門脈合併膵頭十二指腸切除症例１０８例のうち本研究の基準を満たす症例87例について、左側門亢症の典型的な症状である術後腹腔内静脈瘤の形成頻度を見てみると、43例 （49.4％）に発生していることが分かった。うち4例で静脈瘤破裂をきたしていることが判明し当疾患の重要性が再認識された。

全国の日本肝胆膵外科学会高度技能専門医制度 認定修練施設（A）に第一次アンケートを行い、2005.1-2014.12の10年間に施行した、膵頭部癌に対する門脈合併PD症例数と血管合併切除を伴わないPD症例数を返答していただき、術後1年以上のフォローアップが行われ、6, 12ヶ月後に造影CTと血液検査が施行されている門脈合併PD症例を10例以上有し、アンケートに協力可能な施設を抽出。

We distributed the the first questionaries to the A ranked hospitals authorized by the Japanese society of hepatobiliary pancreatic surgery and selected the institutions in which PD with PV resection were carried out at least 10 cases from 2005.1 to 2014.12.
