簡易自己血糖測定器（SMBG）を使用している糖尿病患者の健康寿命の延伸と生活の質（Quality of Life, QOL）の改善のために，SMBGを有効活用した糖尿病療養指導モデル（positive SMBG）を確立する．

選択基準/Key inclusion criteria

1）SMBGを3か月以上実施している外来1型および2型糖尿病患者
2）質問票への回答が十分可能と主治医が判断した患者
3）年齢20歳以上，90歳未満

1)patients with type 1 or type 2 diabetes mellitus using SMBG for moer than 3 months.
2)patients whose doctors confirm eligible for answering questionnaires of the study.
3)patients who aged 20-90 years.

除外基準/Key exclusion criteria

日本語
1)認知症や精神疾患への罹患が強く疑われる患者
2)研究実施責任者又は研究分担者が本臨床研究に不適当と判断した者
