角膜の最深層にあたる角膜内皮細胞が障害される水疱性角膜症に対しては、現在は角膜内皮部分のみを選択的に交換する角膜内皮移植術が標準的治療となっている。ドナー組織に角膜実質を含むDescemet stripping automated endothelial keratoplasty (DSAEK)と、単層の内皮細胞のみのDescemet membrane endothelial keratoplasty (DMEK) が主流である。これらの術式では、1世紀前の全層角膜移植術 (Penetrating keratoplasty) と違い、高い術後視機能が期待できる1)。術後視力に関しては、多くの報告があるが、患者の自覚的な視力の改善、生活の質との関連については評価されていない。今回、NEI VFQ-25（The 25-item National Eye Institute Visual Function Questionnaire）を用いた研究を考案した。

Endothelial keratoplasty, such as Descemet stripping automated endothelial keratoplasty (DSAEK), or Descemet membrane endothelial keratoplasty (DMEK), is expected to provide better visual function. However, the correlation between visual acuity and the quality of vision has not been well understood until now. We have to analyze the quality of life after endothelial keratoplasty using the NEI VFQ-25 subscale score.
