目的1/Narrative objectives1

日本語
統合失調症を始めとする重症精神疾患の患者さんたちが、メンタルヘルスサービスを受けることができていない現状があり、低・中所得国で統合失調症を中心とした重症精神疾患患者さんへの取り組みが重要な課題として取り上げられた。
日本では、統合失調症を含む重症精神疾患を持つ患者さんの治療のために、医師、看護師、臨床心理士、ソーシャルワーカー等を含む多職種チームで地域生活支援を行うことの有効性が示されており、本介入研究においては、低中所得国において、その技術を現地の実情に合わせながら移転・導入することの有効性を検証する。また、その有効性を検証するための簡便な精神症状評価尺度を開発し、低中所得国と高所得国との間のメンタルヘルスギャップを改善することを目的とする。また、アンケート調査では、SUMHが前述の地域精神保健活動を行なっている地域の住民に対して、活動開始前に行われたものと同じ精神保健に関する意識調査を行い、前後の結果を比較することにより、低中所得国における地域精神保健活動の有効性を検証する。
これらの研究により、低中開発国における統合失調症を中心とした重症精神疾患患者さんに対する地域精神保健活動の有効性が立証され、低中所得国におけるこのような取り組みが促進されることによって、重症精神疾患患者さんの福利厚生が改善することが期待できる。

Patients with severe mental disorders centering on schizophrenia, have not been able to receive mental health services in low- and middle-income countries . Efforts have been made on this important issue.
In Japan, for the treatment of patients with severe psychiatric disorders including schizophrenia, the effectiveness of supporting regional living, with multidisciplinary teams including doctors, nurses, clinical psychologists, social workers, etc., has been shown . In this intervention study, we verify the effectiveness of relocating/introducing technology in low- and middle-income countries according to local circumstances. We aim to develop a simple mental symptom evaluation scale, to verify its effectiveness, and to improve the mental health care gap between low- and middle- income countries and high-income countries. In addition, in the questionnaire survey, researchers will conduct the same mental health consciousness survey done by SUMH on the above-mentioned community mental health activities before the start of activity, and compare the results before and after in order to validate the effectiveness of community mental health activities in low- and middle-income countries.
These studies will demonstrate the effectiveness of community mental health services for patients with severe mental disorders centered on schizophrenia in low- and middle-income countries, and by promoting such efforts in low- and middle-income countries, it can be expected that the well-being of patients with severe psychiatric disorders will improve.

１）ICD-10(WHO)による診断基準で統合失調症と診断された者
２）同意取得時に16歳～65歳の者

1) Persons diagnosed with schizophrenia based on ICD-10 diagnostic criteria (WHO);
2) Persons between 16 and 65 years old when giving consent.

除外基準/Key exclusion criteria

日本語
１）過去に意識消失を伴う頭部外傷の既往のある者。
２）中枢性神経疾患（てんかん・炎症性疾患・変性疾患・脳血管障害を含む）の診断の有る者、あるいはその疑いのある者。
３）重篤な心疾患・呼吸器疾患・消化器疾患・腎疾患があり、全身状態が不良の者。
４）悪性腫瘍の診断があり、現在治療を受けている者。
５）妊娠中の者、またその疑いのある者。
６）授乳中の者。
７）その他、担当医により本研究への参加が不適当と判断された者。

1) A history of head injury accompanied by loss of consciousness in the past;
2) A diagnosis of central nervous system disease (including epilepsy, inflammatory disease, degenerative disease, cerebrovascular disease), or suspicions of such a disease;
3) Severe heart disease, respiratory disease, gastrointestinal disease, kidney disease, or poor general health;
4) A diagnosis of malignant tumor and currently receiving treatment;
5) Pregnant or suspected to be pregnant;6) Lactating;
7) Others deemed inappropriate for participation in this research by the attending physician.
