I. 尿中糖鎖シグナルを主とする各種バイオマーカーによる
①「IgA腎症」と「その他の原発性糸球体疾患」の鑑別診断
②「糖尿病腎症」と「糖尿病腎症以外の腎臓病」の鑑別診断
　　　　　③「腎硬化症」と「腎硬化症以外の腎臓病」の鑑別診断
　　　　　④「肥満関連腎症」と「肥満関連腎症以外の腎臓病」の鑑別診断
　　　　　⑤「膜性腎症」と「膜性腎症以外の腎臓病」の鑑別診断
　　　　　⑥「微小変化型ネフローゼ症候群」と「微小変化型ネフローゼ症候群以外の腎臓病」の鑑別診断
　　　　　⑦「ループス腎炎」と「ループス腎炎以外の腎臓病」の鑑別診断
　　　　　⑧「ANCA関連血管炎」と「ANCA関連血管炎以外の腎臓病」の鑑別診断
II. IgA腎症における腎・心血管・生命予後と尿中糖鎖シグナルを主とするバイオマーカーとの関連の検討
①腎生検時からのeGFR 30-40%低下(以降も継続的にこれ以下の低下を満たす場合)
②腎生検以降のeGFRの低下速度が3～5 mL/min/1.73m2/year以上
腎生検時とそれ以降の経時的(年1回以上の頻度)なeGFRを用いて線型回帰曲線を
作成しその傾きとして定義するeGFR低下速度
③末期腎不全による透析導入もしくは腎移植
④アルブミン尿/蛋白尿によって規定される臨床病期の進展(正常アルブミン尿から微量アルブミン尿への発展など)
⑤生命予後
⑥心血管合併症エンドポイント：入院を要する、心イベント(不整脈・狭心症・心筋梗塞・心不全)、脳血管イベント(脳梗塞・脳出血)、大血管イベント(閉塞性動脈硬化症)
III. IgA腎症の病理組織学的所見と尿中糖鎖シグナルや各種予後との関連の検討

I Urinary glycan biomarker for investigatiing differencial diagnosis

8. To differenciate ANCA-associated vasculitis from other kideny disease

II Investigation of the relationship of urinary glycan profile with renal outcome, cardiovascular event, and mortality.
1. eGFR 30-40% decline from baseline
2. eGFR decline > 3-5 ml/min/1.73m2/year
3. Commencement of dialysis or renal transplantation because of end stage renal disease
4. Development of albuminuric stage
5. Mortality
6. Cardiovascular event for admission

III. Examination of the association between histopathologic parameters and urinary glycan excretion

腎生検施行患者
1. 2000年12月1日～2017年9月30日までの間に岡山大学病院で腎生検を施行され腎病理診断が確定した患者
2. 岡山大学病院における過去の臨床研究「血中・尿中・腎組織における糖鎖不全IgA検出による慢性腎臓病(IgA腎症および他の腎症)の新規診断法の開発」(倫1009)または「血中，尿中，腎組織における，Semaphorin3A，TFF，5MedCyD，Klotho検出による腎臓病(検尿異常，腎炎，腎機能障害)の新規診断法の開発」[研1509-005(2063)]の研究に対する同意を文書で得られた患者
3. 性別：男女問わず
4. 年齢：制限なし
健常者①
1. 2017年7月以降、当院で腎移植のドナーとなる人
2. 腎移植前の尿検査で異常所見(尿潜血および尿蛋白)を認めなかった人
3. 腎移植手術時の0時間腎生検の結果で、腎臓病の診断に至らなかった人
4. バイオバンクへの血液・尿検体保管に同意した人
健常者②
1.「糖鎖プロファイリングによる糖尿病合併症における新規バイオマーカーの同定」(研1702-026)の対象健常人として登録された人
2. 検尿異常(尿潜血および尿蛋白)がない人
3. 生活習慣病の合併のない人

Patients with renal biopsy
1. Patients who underwent renal biopsy from December 1, 2000 to September 30, 2017 and were diagnosed with kidney disease.
2. Patients who were enrolled into previous clinical studies of our department(No: 1009 and No: 1509-005[2063]).

Healthy volunteers 1
1. Donors of kidney transplantation in our hospital since July 1, 2017
2. Donors without urine abnormalities such as hematuria and proteinuria.
3. Donors who are not diagnosed with any kidney disease in the results of 0 hour-renal biopsy.
4. Donors who agree to keep their blood and urine sample in OKADAI BIOBANK (Biobank of Okayama university hospital).
Healthy volunteers 2
1. People who were registered as healthy volunteers in a clinical study (No: 1702-026, Okayama university hospital)
2. People without urine abnormalities (hematuria and proteinuria)
3. People without metabolic syndrome
