目的1/Narrative objectives1

日本語
植込型補助人工心臓を装着した患者に対し、SNSを用いた在宅管理システムを用いて、患者と心不全診療チームとの協働による遠隔管理を実施することにより、補助人工心臓の合併症を早期に察知または未然に防止する方法を確立する。

The objective is to prevent complications of left ventricular assist device (LVAD) at an early stage by collaborating with LVAD patient, family member and heart failure team by providing home medical care support using SNS.

　①神経機能障害（脳梗塞・脳出血）の件数
　②ドライブライン感染症の件数
　③出血合併症の件数
　④ポンプ内血栓の件数
(2)その他の合併症（機器のトラブル、右心不全、大動脈弁逆流、不整脈など）の累積発症件数
(3)植込型補助人工心臓による合併症の発症前30日間におけるTTR
(4)全ての原因による死亡数
(5)アドヒアランス
　（実際に測定した日数/測るべき日数）×100(％)
測るべき日数は、1か月の日数から測定不可能な日数を差し引いた値
(6)その他の有害事象の累積発症件数
(7)患者、介助者の心理的負担尺度(HADS)

植込型補助人工心臓装着患者で在宅復帰プログラム※（皮膚貫通部の管理・機器管理・日常生活の自己管理、トラブルシューティング等）を終了し、退院可能と判断された患者、または既に退院している患者

Heart failure patients with LVAD who completed the home management program (self-management of daily life, wound site, devices and troubleshooting) and evaluated as dischargeable or already discharged from the hospital.

除外基準/Key exclusion criteria

(1)電話による連絡が不可能な患者
(2)本人（または代諾者）から文書による同意の得られない患者
(3)その他、担当医師が本研究の対象として不適当と判断した患者

(1) Without access to a telephone line.
(2) Patients who refused to participate in the study
(3) Patients who are considered not eligible for the study by the attending doctor due to other reasons
