2011年3月11日三陸沖を震源とする巨大地震が発生、それに伴う巨大津波が東北地方沿岸部を襲い、未曾有の被害を及ぼした。大災害が先天性疾患の発生に影響を与える可能性はこれまでいくつかの報告がなされている。今回我々は宮城県内における唇裂・口蓋裂罹患率への影響を調査する。

2011年3月11日三陸沖を震源とする巨大地震が発生、それに伴う巨大津波が東北地方沿岸部を襲い、未曾有の被害を及ぼした。大災害が先天性疾患の発生に影響を与える可能性はこれまでいくつかの報告がなされている。2005年にアメリカ　ニューオリンズを襲ったハリケーンカトリーナでは口唇裂・口蓋裂の罹患率の上昇が報告されて、避難状況下のストレスや劣悪な環境的要因が原因と推察されている。今回我々は、東日本大震災が宮城県内における口唇裂・口蓋裂の罹患率へ与えた影響について調査し、大災害が先天性疾患の発生に影響を与える可能性について検討する。

主要アウトカム評価項目/Primary outcomes

日本語
集計したデータと同時期の宮城県内出生数を用いて各群の罹患率を計算する。宮城県内出生数は政府統計web site 「e-stat」を利用参照する。対照群での罹患率は、他家の報告する本疾患の罹患率と比較し、その信頼性を評価する。
各群間の罹患率をChi-squar testで統計学的に比較検討を行う。
各群の月別罹患率を同様に算出し、震災影響群の月別の傾向を観察する。罹患率が突出する月がある場合は、その変動が有意なものなのかを対照群の月別罹患率のバラツキとunpaired t testにて統計学的に比較検討を行う。

Considering that organogenic period was most sensitive to factors of circumstances, the affected period of birth was defined as during a year after 9 months of the disaster. The control period was setting as during 5 years before the affected period.
Main outcome measures were incidences of cleft lip/palate during the control period and the affected period by the Great East Japan Earthquake and Tsunami.

口唇の形成期が胎生4－7週であることから、震災後の環境変化が影響を及ぼす可能性があるのは震災後9ヵ月以降に出生した児であると考え、東北大学病院形成外科および宮城県立こども病院を受診した患者中、2011年12月1日以降1年間に出生した口唇裂・口蓋裂児を震災影響群とする。

Annual and monthly numbers of births with cleft lip/palate patient were obtained from the medical records of division of plastic surgery at Tohoku university hospital and Miyagi children's hospital. Considering that organogenic period was most sensitive to factors of circumstances, the affected period of birth was defined as during a year after 9 months of the disaster.

除外基準/Key exclusion criteria

宮城県内で出生したものの県外へ出生届が出される患児（いわゆる里帰り出産児）と、新生児期に必ずしも診断がつかない粘膜下口蓋裂症例は対象から除外する。

疾患の発生率（1万出生あたり）を対照群vs震災影響群およびp値で示す。
唇裂及び唇顎口蓋裂では13.8 vs 16.7; p=0.342
口蓋裂単独では5.2 vs 3.2; p=0.267
口唇裂・口蓋裂全体では19.0 versus 19.9; p=0.799
と統計学的に有意な上昇を認めなかった。

Results: There were no significant differences between the control and affected periods in the prevalence (per 10,000 live births) of cleft lip with or without palate (13.8 versus 16.7; p=0.342), isolated cleft palate (5.2 versus 3.2; p=0.267), or overall cleft lip and/or cleft palate (19.0 versus 19.9; p=0.799).
Conclusions: We did not observe that the 2011 Tohoku earthquake and tsunami affected the birth prevalence of CL/P in Miyagi prefecture, even though it severely impacted human health in the area.

後ろ向きコホート研究である。
対象は、東北大学病院形成外科、あるいは宮城県立こども病院を受診した口唇裂・口蓋裂児のうち、2011年12月1日以降1年間に出生した児を震災影響群とし、2006年1月1日から2010年12月31日までに出生した児が対照群とし、生まれた年、月、性別、疾患名のみが登録される。

Design: This was a retrospective cohort study.
Participants: The annual and monthly numbers of infants born with CL/P were obtained from medical records. The affected period of birth was defined as one year from December 1, 2011 to November 30, 2012. The control period was 5 years from January 1, 2006 to December 31, 2010. The annual and monthly numbers of births in Miyagi prefecture were obtained using e-Stat, which is a portal for Japanese government statistics.
Main outcome measures: Main outcome measures were birth prevalence of CL/P during the control and affected periods.
