・口腔ケアの回数や手順が記載された独自の口腔ケアプロトコールを導入する。
・口腔ケアプロトコール導入前後の入院時、１週間後をOHATで口腔評価を行う。
・患者は、BDR指標で「歯磨き」「義歯着脱」「うがい」のいずれかに介助を要する急性期脳卒中患者とする。
・口腔評価の評価者は、口腔ケアプロトコール前は、OHATの評価を学習した研究担当看護師で、口腔ケアプロトコール後は、口腔ケアプロトコールとOHATを学習したSCU看護師で行う。

We introduce an original oral care protocol which describes frequency and procedure of oral care.
We evaluate patient's oral cavity at the time of admission and one week later with OHAT.
We examine the patients with acute phase stroke who depend for "tooth brushing","denture attachment / deattachment" or "gargle" on BDR index.
Evaluators of oral cavity evaluations are the research nurses who studied the evaluation of OHAT before the oral care protocol and after the oral care protocol they are done by the SCU nurse who studied the oral care protocol and OHAT.

福岡大学筑紫病院SCUでは、約3カ月で50～60名の新規入院患者がおり、入院時にBDR指標が「歯磨き」「義歯着脱」「うがい」のすべてにおいて自立の患者もいるため、入院中のBDR指標で「歯磨き」「義歯着脱」「うがい」のいずれかが一部介助や全介助の脳卒中の急性期患者で患者や家族から同意説明文書に同意が得られた対象者50名とする。また、基本は入院初日とするが、緊急手術や夜間の入院に関しては翌日評価日とする。

除外基準/Key exclusion criteria

入院時にBDR指標の「歯磨き」「義歯着脱」「うがい」がすべて自立となった患者や入院時と入院後1週間を評価できなかった患者である。
