介入の目的/Purpose of intervention

介入1/Interventions/Control_1

日本語
介入群は、入院期心臓リハビリテーションと並行して神経筋電気刺激療法を30分／日、5日／週の頻度で2週間実施する。刺激部位は大腿四頭筋と下腿三頭筋とし、刺激強度は膝関節の進展と、足関節の底屈を視覚的に認める低度とする。

In the intervention group, patients receive inpatients cardiac rehabilitation and neuromucular electrical stimulation on the bilateral quadriceps femoris and triceps for 30 minutes, five times a week, for 2 consecutive weeks. The intensities of stimulation is set to induce knee extension and ankle planter flexion.

対照群も介入群と同様に入院期心臓リハビリテーションと並行して神経筋電気刺激療法を30分／日、5日／週の頻度で2週間実施するが、刺激強度は刺激を感じる程度で筋収縮を認めない程度とする。

選択基準/Key inclusion criteria

急性心不全あるいは慢性心不全急性増悪にて入院し心臓リハビリテーションの対象となった成人心不全患者で研究参加に同意を得られた連続症例

All consecutive adult patients who are hospitalized with decompensated heart failure and receive cardiac rehabilitation at Hirakata kohosai Hospital

除外基準/Key exclusion criteria

日本語
・入院前より屋内歩行が自立していない症例
・入院後初回歩行にて10m自由歩行速度が≧0.83m/secの症例
・精神疾患患者
・認知症・せん妄・不穏によりNMES施行中の安静の維持が困難な症例
・閉塞性動脈硬化症（Fontein分類≧Ⅲ）
・機械的補助循環装置（IABP・PCPS）装着中の患者
・神経筋疾患の既往がある症例

-Patients who are not able to walk prior to admission
-Patients who are able to walk 10 m at speed of 0.83 m/sec or more
-Patients with psychiatric disease
-Patients who are not able to receive neuromusclar electrical stimulation due to severe dementia, delirium, or agitation
-Patients with peripheral arterial disease
(Fontaine classification III and IV)
-Patients with mechanical circulatory support
-Patients with neuromuscular disease
