日本語
乳児期のアトピー性皮膚炎（AD）は食物アレルゲン感作や食物アレルギー（FA）の原因となることが明らかになっている。乳児期ADの予防には保湿薬によるスキンケアが重要であるが、その効果は十分ではない。本研究では、新生児期からバリア機能を破綻しづらい洗浄・保湿を行って湿疹を予防できる児とできない児の相違をあきらかにする目的で、出産後から生後12ヶ月までの乳児について、皮疹の有無、皮膚バリア機能、皮膚・唾液・便の細菌叢・血中・母乳中の免疫活性物質、アレルゲン感作などを経時的に調査する。これにより、AD発症に関連する母体側・乳児側のINCIを明らかにし、AD・FAの予防に役立てる。

Barrier dysfunction associated with atopic dermatitis (AD) in the infancy is one of most important factors for food allergen sensitization and food allergy (FA). It was reported that some moisturizing agents reduced incidence of AD in infancy. However, its effect may not be sufficient. The aim of this study is to elucidate factors related with AD in infancy by comparing the children who develop AD and those who do not develop AD despite of cleaning and moisturization to keep barrier function from neonatal period until 12 months of age.
Transepidermal water loss, immunoactive substances in sera and breast milk, bacterial flora of skin, feces and saliva are examined at several time points from birth until 12 months of age.

(1)洗浄と保湿によるスキンケアを行った乳児の生後12か月までのアトピー性皮膚炎の発症率を調査し、過去のデータ比較する（historical control）。
(2)上記(1)のコホートについて、経時的に皮膚の観察、アレルゲン感作、唾液・皮膚・便の細菌叢、血中・母乳中免疫活性物質を解析し、アトピー性皮膚炎発症に関連する母体側・乳児側因子を明らかにし、今後のアレルギー疾患の予防に役立てる。

(1)Investigate the incidence of atopic dermatitis up to 12 months of age in infants whose skin care was done by washing and moisturizing, and compare past data (historical control).
(2)About the cohort of (1)above, development of AD, allergen sensitization, the bacterial flora of saliva/skin/stool, immunoactive substance in breast milk and sera will be evaluated.Based on these studies factors on the maternal side and infant side related to development of AD and sensitization will be found and used for future prevention of allergic diseases.
