日本語
我が国における2015年のがん統計予測（国立がん研究センター発表）によると大腸がんの罹患数は135,800人とがんの中で最多となり、死亡数も50,600人と肺がんについで2番目に多い。大腸がんは肺がんと並んで増加の一途を辿っているが、発見される大腸がんのうち進行癌の占める割合は60%前後と診断の遅れが問題となっている。また、大腸がんの５年生存率（2004～2007年全がん協加盟施設の生存率共同調査）はStageⅠ～Ⅲでは80％以上なのに対し、StageⅣでは20％以下であることから、大腸がんにおいては、早期診断と治療により予後の改善が期待できる。またほとんどの大腸がんは、10年以上の長い経過の中で、腺腫からがんへと進展していくことが知られており、特にがん化のリスクの高い前がん病変として、1cm以上の大きさを持つ大腸腺腫を診断することも、侵襲の少ない内視鏡治療を可能とし、さらに大腸がんの予後を改善しうる有力なアプローチであると考えられる。
大腸がんを早期発見するためのスクリーニング検査としては、1次検診として一般に免疫法便潜血検査が行われ、陽性反応が出た場合には大腸内視鏡による精密検査が行われる。しかし、2013年の１次検診受診率は35.4%(全国平均)と低い。また免疫法便潜血検査の問題として、疑陽性率が高く、また早期大腸がんに対する感度が約50％と低いことが挙げられる。そのため、より簡便かつ高精度な、高リスク大腸腺腫を含む大腸がん診断テストの開発が望まれている。米国においては、最近免疫法便潜血検査と便中DNAを組み合わせた診断キット（Cologuard）がFDAに承認された。Cologuardの高リスク大腸腺腫に対する感度は42.4％である (Imperiale et al. NEJM 2014) 。血液を用いた大腸がん診断は、その簡便さと受容性の高さから非常に有望ではあるが、Cologuard同様最近FDAに承認を受けた、血中メチル化SEPT9検査の高リスク大腸腺腫に対する感度は11.2％と低い(Church et al. Gut 2014)。そこで今回、MD Andersonがんセンターと共同で、血漿中タンパク質、自己抗体、マイクロRNA、エクソゾームの網羅的解析から、高リスク大腸腺腫と早期大腸がんの新規血液バイオマーカの探索同定を行い、大腸がんスクリーニングに有用なバイオマーカーパネルの確立を目指す。

In japan, colorectal cancer ranks 1st in all cancer, but prevalence of fecal immnological test(FIT) is still around 30%, and even in examinees with positve FIT, only 50-60% of examinees undergo colonoscopy. In US, cologuard and sept9 have been approved by FDA, but they are not so sensitive in advanced adenoma and early-stage adenocarcinoma. In this study, we determine to identify the blood biomarker which detect advanced adenoma and early stage adenocarcinoma.

選択基準/Key inclusion criteria

大腸内視鏡被験者で、高リスク大腸腺腫（1cm以上、高異型腺腫、絨毛腺腫）・早期大腸がんにて内視鏡切除を行う患者。

Patients with high-grade adenoma and early-stage adenocaricnoma with >9mm in size who will undergo endoscopic resection.

除外基準/Key exclusion criteria

１．妊娠している、または妊娠する可能性のある女性。
２．腸閉塞あるいは小腸・大腸狭窄を疑わせる症状を有する者。
３．その他、研究責任者、分担者が不適切と判断した場合。
