術前呼吸機能低下患者は、術後肺合併症の危険性が高く、特に全身麻酔による筋弛緩薬の使用や人工呼吸による影響が大きいとされている。重度呼吸機能低下患者に対しては、全身麻酔を回避する術式・麻酔方法を選択しているが、術前の呼吸機能検査の結果によって、全身麻酔が可能であるかの判断については明確な基準はない。

今回、過去10年間の外科手術を受けた呼吸機能低下患者において、患者背景、術式・麻酔方法、術後肺合併症を後ろ向きに検討し、術前の呼吸機能検査結果によって全身麻酔可否の判断が可能であるか検討する。また、術後肺合併症の危険因子についても検討する。

術前因子として、呼吸機能検査(%VC、肺活量、1秒量、1秒率)、既往歴、喫煙歴、年齢、採血結果、SpO2値、手術因子として、術式(開腹、腹腔鏡)、手術時間、麻酔時間、麻酔方法についてMann-Whitney検定、カイ２乗検定、ロジスティック回帰分析を用いて、全身麻酔の可否と術後肺合併症の危険因子を検討する。
