目的1/Narrative objectives1

近赤外線分光法(NIRS)は低侵襲で生体内のヘモグロビンの酸素飽和度を連続的に測定することができる。センサー直下の局所的な酸素飽和度(rSO2)をリアルタイムに算出することで、血流低下もしくは虚血状態を早期に感知できる特性をもつ。本研究は、骨盤骨折における殿筋壊死発生予測としてのNIRSの有効性を評価するとともに、骨盤骨折に対するTAEが殿筋壊死発生の原因であるかを検証することを目的とした。

Near-infrared spectroscopy (NIRS) is minimally invasive and enables successive measurement of oxygen saturation of hemoglobin in vivo. With real-time calculation of regional oxygen saturation (rSO2) under the sensor, NIRS can detect early decreased blood flow or ischemic conditions. This study is to evaluate the efficacy of NIRS for predicting the occurrence of gluteal necrosis and to clarify the relationship between TAE and gluteal necrosis in pelvic fracture cases.

対象症例において、INVOS 5100C(Medtronic社)を用いて、両側臀筋のrSO2を測定する。測定部位(センサー貼付部位)は大転子レベルでの左右それぞれの正中部とする。褥瘡を予防するために測定時のみ貼付する。TAE施行群についてはTAE前後の両側臀筋のrSO2を測定する。第1・3・7・14病日の両側臀筋のrSO2を測定する。

The rSO2 of the bilateral gluteal muscles of subjects should be measured with INVOS 5100C (Medtronic). The measurement sites (where the sensors are applied) are the midline regions of both sides at the level of the greater trochanter. To prevent pressure ulcer, the sensor should be applied only at the time of measurement. The group of TAE should undergo measurement of rSO2 of the bilateral gluteal muscles before and after TAE. The rSO2 of the bilateral gluteal muscles should be measured on Days 1, 3, 7, and 14.

不安定型・重度不安定型骨盤骨折(日本外傷学会　骨盤損傷分類2008 )

Adult patients weighing >=40 kg
Those with unstable/severe unstable pelvic fractures (based on the 2008 classification of pelvic injury provided by the Japanese Association for the Surgery of Trauma)

除外基準/Key exclusion criteria

NIRSセンサーが貼付できない症例 (臀部皮膚病変が存在など)
開放性骨盤骨折で臀部正中部にNIRSセンサーが貼付できない症例
安定型骨盤骨折

Patients for whom NIRS sensor cannot be applied (because of the presence of cutaneous lesion on the gluteal region, etc.)
Those for whom NIRS sensor cannot be applied on the gluteal midline regions due to open pelvic fracture
