①成人している
②がんと診断されている
③産婦人科／生殖医療科がん・生殖医療外来の初診前である
④来院目的は妊孕性温存についてである

Participants are eligible if they who aged 20 and more during fertile generation meet the criteria for cancer, and if they have a first consultation appointment of fertility preservation.

除外基準/Key exclusion criteria

以下3点のうちいずれかに該当する；
①産婦人科／生殖医療科がん・生殖医療外来の初診後である
②文書同意が得られない（インフォームド・コンセントが得られない）
③自記式調査（アンケート）を実施することが困難である

Exclusion criteria are to finish the first consultation of fertility preservation, to be unable to fill in the questionnaire, and to refuse to sign the consent form.

がん患者が妊孕性温存の相談目的でがん・生殖医療外来を初めて受診した時に、研究の趣旨を説明し同意を取得する。外来受診前から妊孕性温存の意思決定をするまでの約1か月間を調査期間として、次の3時点で自記式アンケートを実施する。調査時点１：妊孕性温存相談の初診で来院時、診察前に調査の募集と同意取得したのち、第1回自記式アンケートを配布・回収する。調査時点２：初診終了後に、相談中の患者－医師間コミュニケーションに関する自記式アンケート実施する。回収時に調査時点３で記入するアンケートを配布し、意思決定連絡をしたときに記入し、返信するよう依頼する。調査時点３：患者が妊孕性温存をするかしないか意思決定を医療者に対面または電話で伝えた時に、意思決定の理由に関する自記式アンケートに記入し、郵送法で回収する。

Informed consent is conducted at the timing of coming to the hospital in order to have first consultation of fertility preservation. Three questionnaires in approximately one month are scheduled as follows; Time 1) Before first consultation, Time 2) After first consultation, and Time 3) at the timing of decision of fertility preservation.
