1) 食事療法を実施している2型糖尿病患者
2) eGFRが60ml/min/1.73m2未満の当院通院中の２型糖尿病患者のうちSGLT2阻害薬を服用している患者
3) SGLT2阻害薬服用開始２年以上前からeGFRが定期的に測定されている患者
4) SGLT2阻害薬服用開始から２年以上eGFRが定期的に測定されている患者

1) Continuing SGLT2 inhibitors concomitant with diet therapy for more than 2 years
2) eGFR < 60ml/min/1.73m2 at starting SGLT2 inhibitors
3) There are eGFR data for more than 2 years both before and after starting SGLT2 inhibitors

除外基準/Key exclusion criteria

1）1型糖尿病患者
2）重篤な肝疾患の合併
4) 妊婦又は妊娠している可能性のある婦人
5) 重症ケトーシス，糖尿病性昏睡又は前昏睡
6) 重症感染症，手術前後，重篤な外傷がある患者
7) 食事療法を明らかに遵守できない患者
8) 服薬が厳守されていない患者
9)その他，研究責任者又は研究分担者（以下、研究担当者）が不適当と判断した患者

主な結果/Results

日本語
全通院患者2,722人の中から、2年間で20％以上eGFRが低下している患者の中で、その後SGLT2阻害薬を2年以上投与されている患者もしくは、投与されずに、その後2年以上経過を追えた全患者を抽出した。前者17名、後者58名おり、

SGLT2阻害薬をされていた患者のほうが、有意にeGFR低下が抑制され、開始後2年ではeGFRはかえって増加していた。

急性腎障害を含む重篤な有害事象の増加はなかった。
SGLT2iは安全性が高く、2型糖尿病で高度の腎機能障害を有する患者のeGFRのさらなる低下を防ぐことができた。

%DeGFR+2y in patients who were treated with SGLT2i was significantly increased compared with the patients not treated with SGLT2i (2.3 and -21.7%, respectively; P < 0.0001).
There was no increase in serious adverse events including acute kidney injury.
SGLT2i was safe, and prevented further eGFR decline in patients with type 2 diabetes and advanced renal dysfunction.
