経口内視鏡的筋層切開術(peroral endoscopic myotomy: POEM)を受けた食道アカラシア患者およびアカラシア類縁疾患を対象とし、術中に施行した筋層生検(muscle layer biopsy: MB)に対する筋層切除生検(muscle layer resection biospsy: MRB)の病理組織学的評価の有用性について検討する。

選択基準/Key inclusion criteria

日本語
(1)同意取得時の年齢が20歳以上である。
(2)術前に内視鏡、食道透視、食道内圧測定の検査結果を用いて、食道アカラシアおよびアカラシア類縁疾患と診断した患者。
(3)Eckardt symptom score 3点以上の有症状患者。
(4)POEMによる治療を希望した患者。
(5)対象者本人から本研究に対して文書による同意が得られている。

(1) The age at the time of consent acquisition is 20 years old or over.
(2) Patients who are diagnosed as esophageal achalasia and achalasia related diseases by using endoscopy, esophagram and high resolution manometry.
(3) symptomatic patients with Eckardt symptom score 3 or more.
(4) Patients who will undergo POEM.
(5) Document consents are obtained from the subjects prior to this research.

除外基準/Key exclusion criteria

日本語
(1)本人から同意が取得できなかった患者。
(2)20歳未満の患者。
(3)抗血栓薬服用者
(4)前治療としてPOEMや外科的筋層切開術を受けている患者。
(5)研究責任者が本研究の対象として不適当と判断する。

(1) Patients who can not obtain consent from himself / herself.
(2) Patients under 20 years old.
(3) Patients who take antithrombotic drugs
(4) Patients underwent POEM or surgical myotomy before this research.
(5) Patients who are considered inappropriate for this research by the research director.

2019年2月1日から当院でPOEMを施行する連続した39症例の食度アカラシアおよびアカラシア類縁疾患を対象とし、POEM術中に筋層生検および筋層切除生検を行い、採取した検体を比較することで、病理組織学的評価における有用性を比較する。

Consecutive series of patients who will undergo POEM in our hospital from Febrary 1, 2019 are enrolled in this study. The collection of full-thickness muscle layer are performed by using both muscle layer biopsy and muscle layer ressection biopsy among same patients and the usefullness for histopathological examination are compared each other.
