日本語
「休暇を取るほどでもないが、なんとなく調子が悪い、今一つ仕事がはかどらない」という労働者の状況（Presenteeism）が生産性を低下させ、企業に多額の損失を与えていることが2000年代に入ってから指摘され始めた。その原因のうち、多くの労働者が抱える様々な症状（例：肩こり、腰痛、等）による損失総額は、病気による欠勤や、治療費をはるかに上回っていることが判明した。こうした状況に対する包括的な産業衛生管理・健康経営の対策として、鍼治療の施術費用に対する助成が有用であることを検証し、国民の健康保持・増進に寄与することを明らかにしたい。

In the early 2000s, the presenteeism concept, which means that office workers consider to themselves, "I am not so ill as to take a day off from work, but I feel somehow unwell and cannot work as efficiently as I thought," began to be recognized as decreasing labor productivity and inflict large losses to companies. For one thing, the total economic loss due to various symptoms, such as shoulder stiffness and lumbar pain, in many workers was found to far exceed the loss due to their absences from work because of illnesses and medical costs. This study aimed to verify that acupuncture treatment subsidies are useful as a comprehensive measure for industrial health management/health and productivity management and to clarify that such subsidies would contribute to the maintenance and improvement of the public health and welfare.

通常の対策を任意で行うことに加えて、公募したはり師が属する施術所にて観察期間中に施術を受けた際の領収書を観察期間後に提出することによって、最大8,000円まで助成（キャッシュバック）を受けることができる。

(1)最近1か月間に、肩こりや腰痛など、なんらかの不健康な状態によって、「休暇を取るほどでもないが、なんとなく調子が悪い、いまひとつ仕事がはかどらない」と自覚している者
(2)一定の期間に、継続して雇用され、仕事をしている者
(3)主な就業が生産（現場・運搬作業者、製造など）以外の者
(4)調査期間中に2回程度、研究実施場所（豊島区）に来ることができる者

　「肩こり」や「腰痛」などでPresenteeism（欠勤には至っていないが、労働遂行能力が低下している状態）と自覚しているオフィスワーカーに、鍼治療に要した費用に対して4週間に合計8,000円まで助成（キャッシュバック）すると提示すると、1回受療して症状が1/4程度改善し、7,000円の助成を受け、企業が被る損失を一人当たり14,117円補填できる可能性を示した。

Financial aid (up to 8,000 JPY cashback for four weeks) was provided to office workers who were aware of their status of presenteeism (owing to neck and shoulder stiffness and low back pain) to cover the expenses of acupuncture therapy. The workers received 1.0 (median) sessions of therapy during the four-week observation period, with a cashback of 7,000 (median) JPY. Financial aid for acupuncture therapy may help compensate for losses incurred by enterprises in the form of 14,117 JPY per worker a month.

　プレゼンティーイズムと自覚しているオフィスワーカーをWeb広告によって募集した。主な適格基準は、①最近1か月間に、健康上の理由で労働遂行能力が低下していると自覚している者、②前後3か月間に、継続して雇用され、仕事をしている者（非正規の職員・従業員を除く）、③主な就業が「管理・事務・技術」に従事する18歳以上65歳未満の労働者、とした。また、除外基準は、同時期に他の臨床試験や治験に参加している者とした。

Office workers who were aware of the effect of presenteeism on their job performance were recruited through online advertisements. The inclusion criteria were as follows: (1) awareness of the decline in one's job performance due to health issues during the past month, (2) being employed and working continuously for three months before and after enrollment in the study (excluding non-regular employees), (3) being engaged in administrative/clerical/technical jobs, and (4) aged between 18 and 65 years old. The exclusion criterion was participation in any other clinical study or trial during the study period.

参加者の流れ/Participant flow

　2019年1月26日から2020年12月28日までに対象者を募集し、基準を満たし同意を得た211名を介入群103例と対照群108例に割り付けて4週間追跡し、脱落した8名を除く203例について解析した。

The participants of this study were recruited from January 26, 2019, to December 28, 2020. Of the 211 participants who met the eligibility criteria and provided informed consent, 103 (including six participants who could not receive the allocated intervention) and 108 were allocated to the intervention and control groups, respectively. Participants were observed for four weeks. After excluding eight dropouts, 203 participants were included in the final analysis.

有害事象/Adverse events

　鍼治療を含む施術後に生じた有害事象については、累計150回の施術のうち110回（73%）においては認められず、眠気15%、倦怠感12%、施術部位の痛み6%、疲労感3%、症状の悪化0.7%、出血0.7%、血腫0.7%が認められた。なお、気分不良、嘔気については認められなかった。

The adverse events post-intervention included drowsiness (15%), malaise (12%), pain at the therapy-applied site (6%), fatigue (3%), exacerbation of existing symptoms (0.7%), bleeding (0.7%), and hematoma (0.7%). No participants complained of a sick feeling or nausea.

評価項目/Outcome measures

　主要アウトカム評価項目である相対プレゼンティーイズム値については、対照群1.00（四分位範囲: 0.76-1.00）よりも介入群1.00（四分位範囲: 0.86-1.00）の方が有意に高かった（効果量(r)=0.15、P=0.03）。
　対照群における一人当たりのプレゼンティーイズムによるコストは60,655（95%信頼区間: 44,441-76,870）円、介入群における一人当たりのプレゼンティーイズムによるコストは40,055（95%信頼区間: 26,114-53,996）円、一人当たりの鍼治療への助成（キャッシュバック）額は6,483（95%信頼区間: 6,080-6,887）円で、介入（キャッシュバック）により期待できる一人当たりの補填月額は、14,117円であった。

The relative presenteeism score (a major outcome measure) was significantly higher in the intervention group (median 1.00 [IQR, 0.86-1.00]) than in the control group (median 1.00 [IQR, 0.76-1.00]; effect size [r] = 0.15, P = 0.03).
The presenteeism cost per worker in the control group was JPY 60,655 (95% CI: 44,441-76,870), and that in the intervention group was JPY 40,055 (95% CI: 26,114-53,996); cashback per worker of JPY 6,483 (95% CI: 6,080-6,887) and monthly remuneration per worker of 14,117 JPY were expected from the intervention group (cashback).
