日本語
近年、高度肥満患者に対する肥満手術の有効性について、減量手術としてだけではなく糖尿病も含めた代謝改善手術としても世界的に注目されている。
肥満手術の減量効果について詳細な機序は不明な点が多い。
また、手術の介入効果は個人差がある。減量効果に個人差がある理由は不明。
肥満症の発症と進行には、個体そのものの生まれつきの特性である肥満関連遺伝子と、生まれた後の環境因子が後天的に遺伝子の表現形式に影響するエピジェネティクスと、さらに実際の食事量や運動量などの生活習慣の3つの要素が考えられている。
肥満関連遺伝子そのものについては、実際の肥満形成や治療効果においてその役割は大きくないと考えられている。むしろ、実際の肥満形成や治療効果に大きくかかわるのは、エピジェネティクス、すなわちDNAメチル化による遺伝子発現制御機構であると考えられる。

本研究では以下の検討仮説につき検証を行うこととする。
検討仮説；肥満症治療における肥満外科手術による減量効果は、エピジェネティクスによる肥満関連遺伝子の表現形式の変化（DNAメチル化）によるものである。

具体的には、肥満外来患者を対象に肥満外科手術による肥満関連遺伝子メチル化の定量変化を主要エンドポイントとして検証する。

In recent years, the effectiveness of obesity surgery on patients with high obesity has attracted worldwide attention not only as a weight loss surgery but also as a metabolic improvement surgery including diabetes.There are many unclear points in details of the weight loss effect of obesity surgery.In addition, there are individual differences in the intervention effect of surgery. The reason why weight reduction effect differs among individuals is unknown.Three factors are considered for the onset and progression of obesity: an obesity-related gene which is an inherent property of the individual itself, epigenetics which influences the expression pattern of the postnatal environmental factor later, and lifestyle such as momentum I will.It is considered that its role in actual obesity formation and therapeutic effect is not important to the obesity related gene itself.Rather, it is thought that epigenetics, ie, gene expression regulatory mechanism by DNA methylation, is greatly involved in actual obesity formation and therapeutic effects.In this research, we will examine the following discussion hypothesis.The effect of weight loss by obesity surgery in the treatment of obesity is due to the change in phenotype of obesity-related genes by epigenetics (DNA methylation).Specifically, we will examine the quantitative change of obesity-related gene methylation by obesity surgery for the outpatient with obesity as a primary endpoint.

①同意取得時において年齢が20歳以上65歳未満の患者
②同意取得時において肥満外科手術の適応条件を満たす患者
③本研究への参加にあたり十分な説明を受けた後、十分な理解の上、本人の自由意思による文書同意が得られた患者

除外基準/Key exclusion criteria

①重篤な心不全・不整脈を合併する患者
②重篤・治療不十分な基礎疾患のある患者
③内分泌（ホルモン）、他の薬剤等による２次性肥満の患者
④基礎疾患等で肥満外科手術の施行が不適応であると判断された患者
⑤その他、研究責任者及び研究分担者が研究対象者として不適当と判断した者
