選択基準/Key inclusion criteria

以下の基準をいずれかを満たす成人患者を対象とする。入院・外来を問わない。
1）臨床的に慢性肝疾患が疑われる者；肝疾患のどの段階から胸壁・腹壁静脈が検出され得るかの検討を行うために選択する。
2) 既に肝硬変の確定診断された者

Adult patients, regardless of hospitalization or outpatient, who meet either of the following criteria are targeted.
1) Subjects who are clinically suspected of chronic liver disease; it is selected to investigate from which stage of liver disease chest wall / abdominal wall vein can be detected.
2) Subjects who have already been confirmed to have cirrhosis.

除外基準/Key exclusion criteria

以下に抵触する患者は本臨床研究に組み入れないこととする。
1）皮膚に病変があり、画像取得が困難な者
2）研究責任医師が患者として不適当と判断した者

静脈強調イメージ（VWI）は通常イメージ（UI）よりも腹壁静脈瘤（AWV）を有意に描出できた（72％対24％、p =0.0005）。多変量解析では、VWI-AWV grade 2が非治療GOVの存在に関連する独立した因子であることが示された[OR =3.05 (1.24-7.53), p = 0.016]。静脈可視化アプリケーションは、肝硬変およびGOVの存在に関連するAWVを非侵襲的に検出し、VWI-AWV grade 2は未治療GOVの存在に関連する独立した因子であった。

Vein-weighted images (VWI) could significantly visualize abdominal wall variecs (AWV) than unmanipulated images (UI) (72% vs 24%, p =0.0005). Multivariate analysis demonstrated that VWI-AWV grade 2 was an independent factor related to the presence of non-treated GOV [OR=3.05 (1.24-7.53), p=0.016]. The vein-viewing application non-invasively detected AWV related to the presence of cirrhosis and GOV, and VWI-AWV grade 2 was an independent factor related to the presence of non-treated GOV.

参加者背景/Baseline Characteristics

慢性肝疾患の患者100名が前向きに登録された。すべての患者は、登録後3ヶ月以内に食道胃十二指腸検査を受けている患者である。男性71名、女性29名（年齢中央値70.5歳）の肝疾患の原因は、B型肝炎（n=19）、C型肝炎（n=21）、アルコール依存症（n=33）、原発性胆汁性胆管炎（n=3）、自己免疫性肝炎（n=4）、その他（n=20）であった。

One hundred patients with chronic liver diseases were prospectively enrolled. All the patients underwent esophagogastroduodenoscopy within three months of the enrollment. The causes of liver diseases among the 71 men and 29 women (median age, 70.5 yr) included Hepatitis B (n=19), Hepatitis C (n=21), alcoholism (n=33), primary biliary cholangitis (n=3), autoimmune hepatitis (n=4) and others (n=20).

iPhoneの静脈観察アプリケーションを用いて、AWVを評価するために、非侵襲画像（UI）と静脈強調画像（VWI）を撮影した。2名の医師が両画像を独立して評価した。UIとAWVの評価は、グレード0（非検出）、グレード1（わずかに検出）、グレード2（明確）とした。
