選択基準/Key inclusion criteria

日本語
術前診断にて大腸癌同時性肝転移と診断され、以下の選択基準をすべて満たすもの。

2.1　選択基準
1)性別不問
2)同意取得時において年齢が20歳以上の患者
3) 腹腔鏡下大腸・肝同時切除の手術が予定されている患者で、以下の条件を満たしたもの
　・肝転移の最大腫瘍径3cm以内かつ肝転移の腫瘍個数2個以内
　・肝転移の局在がクイノー分類（Couinaud分類）のSegment 2-6
　・肝転移が主要脈管（グリソン1、2次分枝、主肝静脈、下大静脈）に近接しない
　・腹腔鏡下肝部分切除、外側区域切除が可能と判断される症例
　4）本研究の参加にあたり十分な説明を受けた後、十分な理解の上、患者本人の自由意思による文書同意が得られた患者

Patients with primary colorectal cancer with synchronous liver metastases in accordance with the following criteria

1)no gender
2)age over 20 years old
3)Patients who will undergo laparoscopic simultaneous resection of primary colorectal carcinoma and liver metastasis in accordance with the following criteria.
･the maximum tumor diameter less than or equal to 3 cm, only up to 2 liver metastasis.
.liver lesions in segment 2-6 (The Couinaud classification)
･liver tumors without proximity to major vessel
･patients who will undergo laparoscopic partial hepatectomy or left lateral segmentectomy.

4)the patients who gained written consent by the patient's free will with sufficient understanding after receiving sufficient explanation for the participation of this study.

除外基準/Key exclusion criteria

日本語
1）腹腔鏡下大腸・肝同時切除の手術を行うと、長時間手術となることが予想される以下のようなものは、同時切除の対象とはせず除外することとする。
・直腸癌で低位前方切除、超低位前方切除、内肛門括約筋切除術などの術式を要する患者
・肝転移巣の手術が亜区域以上の系統切除（外側区域切除を除く）を要するもの　
2）その他、研究責任(分担)者が研究対象者として不適当と判断した患者

1) When the laparoscopic simultaneous resection could be a long surgery as described below
･low rectal cancer requiring super-low anterior resection or intersphincteric resection (ISR)
.liver metastasis requiring anatomical resection (greater or equal to subsegmentectomy, except for left lateral segmentectomy).
2) Patients judged unsuitable as research subjects

大腸・肝同時切除を一期的に低侵襲手術で行えば、入院期間の短縮、患者の身体的、精神的ストレス軽減に寄与できる。日常診療として行っている腹腔鏡下大腸・肝同時切除の前向き観察研究を行うことで、本手術手技の安全性が検証可能と考えられる。
