日本語
感染症の中でも重症の病態の一つである「菌血症」の診断には血液培養検査が必須であるが、血液培養検査陽性例の一部を占める汚染菌により偽陽性となるため、菌血症を適切に予測することが医療者側に求められている。そこで、我々は入院患者を対象に、80%以上の食事摂取があり、悪寒戦慄がなければ97.6% (539/552人)の患者において菌血症ではないとする簡便な菌血症診断予測ルールを開発した(Komatsu et al JHM 2017)。しかし同研究は入院患者を対象として食事摂取量の評価に看護師の介入を要すため、本ルールが外来診療においても有用である証明が必要と判断した。
「食事摂取量」と「悪寒戦慄の有無」は患者から直接問診で得られる情報であるため、本研究では問診票により得られた患者自己評価による「食事摂取量」と「悪寒戦慄の有無」を用いた菌血症診断予測ルールの有用性を証明することを目的とした。

Although blood culture tests are needed for the diagnosis of bacteremia, which is one of the serious pathological conditions among infectious diseases, it is required for medical doctor to properly predict bacteremia to avoid the diagnostic error based on the false positive results of blood cultures by contaminating bacteria. So we had reported a simple prediction rule for diagnosis of bacteremia in hospitalized patients that patients did not have bacteremia with 80% or more food consumption and without shaking chills (Komatsu et al JHM 2017). However, because this study required nurse intervention to evaluate food consumption, we must prove that this rule also is useful in outpatients.
Since "food consumption" and "presence or absence of shaking chills" are information obtained from patients by direct interviews,we aimed in this study to prove the usefulness of this predicting rule using "food consumption" and "presence or absence of shaking chills" according to the patient self-assessment obtained by the questionnaire.
