選択基準/Key inclusion criteria

日本語
(1) 原発巣からの生検により中咽頭癌、下咽頭癌または声門上癌と病理診断
(2) 内視鏡検査、画像検査（CTまたはMRI、PET/CT）にてTis-T2もしくは深部浸潤のないT3と診断
(3) 局所病変の浸潤が咽頭収縮筋までにとどまる症例
(4) p16陰性中咽頭癌、下咽頭癌、声門上癌ではN0-N2b
(5) p16陽性中咽頭癌ではN0、N1
(6) PS (ECOG)が0か1
(7) 手術前30日以内の血液検査にて下記の条件をすべて満たす症例
　1) 白血球数≧2,000/mm3
　2) 血小板数≧100,000/mm3
　3) AST≦100 IU/L、ALT≦100 IU/L
　4) 総ビリルビン≦2.0 mg/dL
(8) 20歳以上の症例
(9) 説明を受けて本人の自由意思による試験参加の文書による同意が得られた症例

(1) Oropharyngeal, Hypopharyngeal or supraglottic cancer confirmed by pathological diagnosis
(2) Tis-T2 or superficial T3 diagnosed by endoscopy or imaging test (CT, MRI or PET/CT)
(3) Tumor invasion within pharyngeal constrictor
(4) N0-N2b in p16-negative oropharyngeal, hypopharyngeal or supraglottic cancer
(5) N0-1 in p16-positive oropharyngeal cancer
(6) ECOG-PS 0-1
(7) Blood test results meeting the following requirements
1) Leukocyte count < 2,000/mm3
2) Platelate count < 100,000/mm3
3) AST= or <100 IU/L, ALT= or <100 IU/L
4) Total bilirubin= or <2.0 mg/dL
(8) Age= or >20
(9) Providing written informed consent

除外基準/Key exclusion criteria

日本語
(1) 開口障害や頸部進展障害により術野の展開が困難な症例
(2) 臨床的節外浸潤を伴う頸部リンパ節転移を有する症例
(3) 両側頸部リンパ節転移を有する症例
(4) 遠隔転移を有する症例
(5) 抗凝固薬、抗血小板薬を休止困難な症例
(6) 妊娠中または妊娠している可能性のある女性
(7) 研究代表者または研究分担者が本研究への参加が不適切と判断する病態の症例

(1) Poor exposure of surgical field due to trismus or difficulty of neck extension
(2) Extranodal extention of lymph node metastasis
(3) bilateral cervical lymph node metastases
(4) Distant metastasis
(5) Difficulty of discontinuation of anticoagulant or antiplatelet drug
(6) Pregnancy
(7) Unsuitable for inclusion at the discretion of the researchers
