目的1/Narrative objectives1

日本語
中心窩を含む滲出型加齢黄斑変性においては、血管内皮増殖因子（VEGF）阻害剤の硝子体内投与、および光線力学療法（PDT）の有用性が明らかとなっている。しかし、その経過中、治療中であっても広範囲な黄斑下出血または出血性網膜色素上皮剥離を合併することがあり、黄斑下出血に対しては確立した治療法はないため治療内容は症例や施設により様々である。今回、重症黄斑下出血を合併した症例において、出血時の各種検査所見と治療選択、治療予後についての観察研究を行う。同時に、それまでの罹病期間、治療内容、網膜色素上皮剥離の有無等が出血後の予後に影響を与え得るかどうかも検討する。これにより、適切な診療方針および治療法の選択に繋がる有益情報が得られると期待される。

In wet age-related macular degeneration involving the fovea, intravitreous administration of vascular endothelial growth factor (VEGF) inhibitors and the usefulness of photodynamic therapy (PDT) has been demonstrated. However, during the course of treatment there may be extensive submacular hemorrhage or hemorrhagic retinal pigment epithelial detachment even during treatment, and there is no established treatment for submacular hemorrhage and the treatment content is case or it depends on the facility. We will conduct observational research on various clinical findings and treatment options at the time of hemorrhage and prognosis in cases with severe submacular hemorrhage. At the same time, it is also examined whether the disease duration, treatment, presence of retinal pigment epithelial detachment may affect the prognosis after hemorrhage. This is expected to provideuseful information leading to the selection of an appropriate medical care policy and treatment.

1)　2015年4月から2018年9月の間に網膜下出血を発症
2)　中心窩を含む網膜下出血が2乳頭面積以上
3） 眼底写真、光干渉断層計（光干渉血管撮影,　眼底自発蛍光）などの眼科画像所見が得られる
4) 網膜下出血発症後3か月以上経過観察されている

1) Subretinal hemorrhage between April 2015 and September 2018
2) Subretinal hemorrhage including the fovea more than 2 disc area
3) Ophthalmic imaging findings such as fundus photography, optical coherence tomography (optical coherence tomography angiography, fundus autofluorescence) can be obtained
4) More than 3 months have been observed after onset of subretinal hemorrhage

除外基準/Key exclusion criteria

日本語
経過中に、患眼に網膜剥離、糖尿病網膜症、網膜静脈閉塞症、ぶどう膜炎等の網膜硝子体疾患を併発した症例。
データの提供に同意いただけない症例。

Cases in which the affected eye has a complication of retinal detachment, diabetes retinopathy, retinal vein occlusion, uveitis and other retinal vitreous diseases.

主な結果/Results

67%の症例がSMHを生じるまで治療歴がなかった。
36眼（28%）の症例が硝子体手術を、49眼（39％）がガス置換術を、42眼（33％）がVEGF阻害薬による単独療法を施行されていた。
硝子体手術は出血時の視力が不良で、サイズの大きなSRHに選択される傾向があった。
ガス置換術、硝子体手術では術後40%以上の症例がSRHは1乳頭径以内に減少し、最終平均視力も小数視力でそれぞれ0.13から0.23に、0.07から0.18に有意に改善した。

Sixty-seven percent of the patients were treatment naive.
Thirty-six eyes (28%) had undergone vitrectomy, 49 eyes (39%) had undergone gas pneumatic displacement, and 42 eyes (33%) had undergone anti-VEGF monotherapy.
With pneumatic displacement and vitrectomy, SRH decreased to within 1 papillary diameter in more than 40% of postoperative cases.
Final mean visual acuity (decimal unit) significantly improved from 0.13 to 0.23 and from 0.07 to 0.18, respectively.
