選択基準/Key inclusion criteria

以下の基準をすべて満たす方を本研究の対象とする。
① 白岩内科医院の外来に通院している生活習慣病（糖尿病、脂質異常症、高血圧、高尿酸血症のうち1つ以上を有する）患者
② 20歳以上の男女
③ 本研究の参加にあたり十分な説明を受けた後、十分な理解の上、本人の自由意思による文書同意が得られた方

Patients who meet all of the following criteria are included in this study.
1) Outpatients with lifestyle diseases (at least one of diabetes mellitus, dyslipidemia, hypertension, and hyperuricemia) in Shiraiwa Medical Clinic
2) male and female who are aged of 20 years or older when giving their consent
3) patients who provide written consent form to participate in this study after full explanation of the study

除外基準/Key exclusion criteria
