呼吸器外科手術において良好な術後鎮痛は呼吸機能の回復や早期離床に重要である。これまで硬膜外麻酔がgold standardと考えられていたが、超音波ガイド下にparavertebral blockを行うようになり、硬膜外麻酔と同等の鎮痛効果が得られている。
Paravertebral blockを行う際に目標となる肋間にsingle shotで薬剤を投与することが多いが、その付近の肋間に複数回穿刺し、薬剤を投与することでより広い範囲に効果を得られたとする報告が散見される。
しかし薬剤が広範囲に広がることが術後鎮痛に有効であるという報告は少ない。
今回我々は呼吸器外科手術においてParavertebral block multiple injectionを行うことでsingle shotで行なった場合と比較して良好な鎮痛が得られる可能性が高いと考え、ランダム化比較試験を行い術後48時間後までのNRSをアウトカムとしてその有効性を検討する

Good postoperative analgesia in respiratory surgery is important for recovery of respiratory function and early ambulation. Until now, epidural anesthesia was considered to be a gold standard, but now paravertebral blocks are performed under ultrasound guidance, and the analgesic effect equivalent to epidural anesthesia has been obtained.
When performing a paravertebral block, the drug is often administered in a single shot between the target ribs, but it is reported that the effect was obtained over a wider area by puncturing multiple times between adjacent ribs and administering the drug Are scattered.
However, there are few reports that widespread use of drugs is effective for postoperative analgesia.
In this study, we considered that performing paravertebral block multiple injection in respiratory surgery is likely to provide better analgesia than performing a single shot, and conducted a randomized controlled trial 48 hours after surgery. The effectiveness of the NRS until the outcome

両側同時手術の患者
術後鎮痛に硬膜外麻酔が適切であると判断した患者
穿刺部の皮膚に穿刺するのに適さない病変がある患者
局所麻酔薬にアレルギーの既往がある患者
手技の施行が危険と考えられる患者
腎機能障害を有する患者
喘息発作の既往を有する患者
その他、研究責任者が被験者として不適当と判断した患者
