本研究の目的は、ヒト炎症性発癌症例の外科摘出標本から物理的に粘液層を除去することにより、大腸粘膜表面および癌組織表面に定着する特異的な細菌叢を16S rRNA遺伝子解析で明らかにすることである。

SutterellaとEnterobacteriaceaeは粘膜を覆う粘液において、粘膜と比較して有意に濃縮されていた。Rikenellaceaeは、がん組織を覆っている粘液において、同じがん組織と比較して有意に濃縮されていた。Ruminococcaceae、EnterobacteriaceaeおよびErysipelotrichaceaeは、がんを覆う粘液と比較して、粘膜を覆う粘液に有意に濃縮されていた。Fusobacteriumは粘膜を覆う粘液と比較して、がんを覆う粘液に有意に濃縮されていた。

A total of 18 CRC patients who underwent primary tumor resection between February 2020 and January 2021 at the Fujita University Hospital were enrolled. Patients who received preoperative chemotherapy or radiotherapy were excluded. Patients with familial adenomatous polyposis or inflammatory bowel disease were excluded. None of the patients had received antibiotics within a month before surgery.

評価項目/Outcome measures

滅菌生理食塩水による高圧洗浄によって粘液が除去されたかどうかを評価するため、Wilcoxon符号順位検定によって洗浄前と洗浄後の粘液深度を比較した。粘液と洗浄した組織サンプル間の分類群の相対存在量の比較も、ウィルコクソンの符号順位検定で評価した。少なくとも1つのグループで相対存在量の中央値が0.01を超える系統または属のみがこの分析に選択された。多重比較を考慮するため、偽発見率をコントロールするBenjamini-Hochberg法を用いた。アルファ多様性の評価にはシャノン多様性指数を用い、Wilcoxon符号順位検定によって粘液サンプルと洗浄組織サンプルの間で再度比較した。β多様性のPERMANOVA比較は、Rパッケージvegan [34]を用いて実行した。統計解析はIBM SPSS Statistics 27（日本アイ・ビー・エム株式会社、東京、日本）を用いて行った。P値が0.05未満を統計的に有意とみなした。

The mucus depth was compared between before and after washing by Wilcoxon signed-rank test to evaluate whether mucus was removed by high-pressure washing with sterile saline. Comparisons of the relative abundances of taxa between mucus and washed tissue samples were also assessed by the Wilcoxon signed-rank test. Only phyla or genera with a median relative abundance >0.01 in at least one group were selected for this analysis. The Benjamini-Hochberg method for controlling the false-discovery rate was used to account for multiple comparisons. The Shannon diversity index was used to evaluate alpha diversity and was compared between mucus and washed tissue samples once again by the Wilcoxon signed-rank test. PERMANOVA comparisons of beta diversity were run using the R package vegan [34]. Statistical analyses were performed using IBM SPSS Statistics 27 (IBM Japan Inc., Tokyo, Japan). P values less than 0.05 were considered statistically significant.
