目的1/Narrative objectives1

本研究の目的は、糖尿病患者の中で異常知覚、知覚鈍麻などの感覚神経障害をきたしている症例の中から足根管での絞扼性神経症状を原因とした神経障害を抽出し、足根管レベルでの絞扼性神経障害に対する神経除圧術の知覚障害への有効性を検証することである

The clinical research consists of patients with neuropathy at the tarsal tunnels due to entrapment syndrome. They will be selected from among diabetic patients with sensory nerve disorder such as paresthesia or hypaesthesia in treatment by Uemura, at Plastic Surgery, Saga University Hospital. The projects objective is to ascertain, using the clinical study protocol, the positive effect that neurological compression will have on tarsal tunnel paresthesia caused by entrapment syndrome

Preoperatively the complain of tingling in the affected foot, positive Tinel-like signs, and sensory disturbance and abnormal motor nerve conduction velocity existed in all feet.The modified Toronto Clinical Neuropathy Score (mTCNS) was used to measure changes before and three months after surgery

選択基準/Key inclusion criteria

日本語
1.糖尿病の診断を受け、自覚症状として足部の感覚異常（ムズムズ、灼熱、無知覚など）を訴える方
2.糖尿病合併症重症化予防のフットケアを実施している経過観察が可能な患者で手術希望がある方
3.術前に患側の脛骨神経伝達速度で低下が認められる方
4.血糖コントロールの投薬治療を受けており、血糖のコントロールが良好である方
5.十分な下肢の血流がある方（足背動脈と後脛骨動脈が触知可能、触知できない場合はABIが0.7以上）

本人による文書での同意を原則とするが、何らかの理由により本人からの文書同意が取得出来ない場合は、代諾者（法定代理人）同意も可能とする

1.The patient had to have symptomatic diabetic neuropathy with clinical signs, such as tingling and touch sensation.

2.The patients took proper care of their feet and want to have the surgery.

3.The patient's neuropathy had to be documented by nerve conduction studies with electrophysiological test results that showed decreased motor nerve conduction velocity before surgery.

4.The patient's diabetes was in good glycemic control.

5.The patients had no arterial deficiency.

Patients get informed consent for this surgery. If impossible to be agree by technical problems families get informed consent.

除外基準/Key exclusion criteria

日本語
1.未治療の糖尿病患者
2.脊柱管狭窄症候群、腰椎ヘルニアなど下肢の神経麻痺にかかわる基礎疾患患者
3.重症虚血肢であり、足背動脈もしくは後脛骨動脈がサウンドドップラーで聴取しない患者
4.放射線療法、化学療法、血管内皮増殖因子阻害剤、ステロイド全身投与などの加療中の患者
5.全身状態、栄養状態が悪い症例、出血性疾患および全身的な膠原病(SLE含む)など原疾患の理由により治療が困難な症例
6.妊娠中または授乳中の患者、小児患者は原則除外する
7.継続観察のために来院することができない患者
8.治療担当医師が治療対象として不適当と判断した患者

4.The patients had treatments by radiotherapy, chemotherapy etc.

5.The patients had poor general condition and nutrition.

6.The patients was pregnant woman and child

7. The patients cannot move to clinic to see the doctors.

8.The patients that were excluded by medical problems.
