この研究の目的は脳卒中発症後早期からKAFOを使用することに妥当性があるのか下肢筋緊張、痛み、日常生活活動により検証することです。

脳卒中発症後早期からの長下肢装具使用が脳卒中患者の身体活動に与えている影響を明確にすることで、長下肢装具使用の利点、欠点が明らかになれば、作成時期や使用期間を判断する指標になるだろう。

The purpose of this study is to examine the validity of using KAFO early after the onset of stroke by examining lower limb muscle tone, pain and daily activities.
By clarifying the effects of the use of lower limb orthoses on the physical activity of stroke patients early after the onset of stroke, if the advantages and disadvantages of using lower limb orthoses become clear, determine the timing of preparation and duration of use It will be an indicator.

測定部位　距腿関節　底屈‐背屈、膝関節　屈曲‐伸展、肘関節　屈曲‐伸展
測定時期　理学療法実施日における理学療法開始前および終了後　
MTSは、関節可動域(Range of Motion; ROM)と筋の反応の質(Quality of Muscle Reaction; QMR)を測定する痙縮評価指標で、測定肢位と筋の伸張速度が規定されているのが特長である。この測定方法の臨床的有用性は竹内8）らにより報告されている。MTSでは関節可動域を測定するが、1度単位で測定できるデジタル角度計を使用する。

：装具装着時から2週ごと退院まで。

選択基準/Key inclusion criteria

①　大西脳神経外科病院に入院し医師より理学療法処方のあった初発脳卒中（脳梗塞、脳出血、くも膜下出血）患者
②　装着時期は装着適応表を基に決定
③　意識障害　ＪＣＳ一桁以下
④　口頭による指示が理解できる
⑤　座位可能（介助があってもよい）
⑥　非麻痺側下肢筋力　ＭＭＴ4以上

1.First stroke patient (cerebral infarction, cerebral hemorrhage, subarachnoid hemorrhage) who was admitted to Onishi Neurosurgery Hospital and was prescribed physiotherapy
2.The wearing time is decided based on the wearing adaptation table
3.Consciousness disorder JCS single digit or less
4.Understand verbal instructions
5.Sitting is possible (may be assisted)
6.Non-paretic side lower limb muscle strength MMT4 or higher
